盐酸氢吗啡酮复合咪达唑仑镇痛镇静在成人眼底手术中的应用 被引量:8

摘要 目的:探讨盐酸氢吗啡酮复合咪达唑仑在成人眼底手术中的镇痛和镇静效果。方法:80例成人择期球后阻滞局部麻醉眼底手术患者,随机分为两组:A组41例,应用氢吗啡酮0.25mg及咪达唑仑1.5mg加入0,9%生理盐水稀释成2ml;B组39例,给于0.9%生理盐水2ml.两组患者均由术者进行球后阻滞局部麻醉。术中监测HR、MAP、Sp O2、RR、脑电双频指数(BIS)、镇静程度评分(Ramsay评分)、患者合作评分、术者满意度,术后随访患者满意度和不良反应发生情况。结果:两组患者术前基本情况差异无统计学意义。术中A组HR、MAP及BIS值低于B组(P<0.05);两组RR和Sp O2差异无统计学意义(P>0.05)。A组镇静Ramsay评分、合作评分、满意度评分、球后阻滞疼痛评分明显优于B组(P<0.05)。两组围术期均无严重不良事件发生。结论:盐酸氢吗啡酮复合咪达唑仑可安全用于球后阻滞眼底手术。 Objective: To explore the advantage of analgesia and sedition by taken with hydromorphone combined midazulum in adult retinal surgery.Methods: Eighty adult patients, ASAⅠ~Ⅱ, who underwent elective retinal surgery, were randomly divided into two groups based on the local annesthesia for basic analgesia:A group(n=41): hydromorphone 0.25mg combined midazulum 1.5mg ;B group (n=39):0.9%sodium chloride 2ml. All patients received local anesthesia by surgeon himself. The BP, HR, SpO2, RR, BIS ,Ramsay and cooperation scores were measured during the operation, and the satisfactory scores and side effects were recorded after the operation.Results:There was no signiifcant difference in BP, HR, SpO2 and BIS in all patients before surgery(P〉0.05).The HR,MAP and BIS in group A was lower than that in group B(P〈0.05); there was no signiifcant difference in RR and SpO2 in two groups (P〉0.05).The Ramsay, cooperation and satisfactory scores, analgesia score after local anesthesia in group A were higher than that in group B(P〈0.05). No adverse event was observed in two groups.Conclusion:The analgesia and sedition by taken with hydromorphone combined midazulum are an ideal and safe way to used in adult retinal surgery.
