摘要
目的：探讨踝肱指数（ABI）对住院糖尿病（DM）患者DM足预防的作用。方法：回顾性分析我院收治的268例2型DM患者的临床资料,按ABI将患者划分为3组,A组（正常组）：236例,ABI0.9~1.3;B组（增高组）：5例,ABI≥1.3;C组（下降组）：27例,ABI〈0.9;对各组患者临床特征、ABI异常率分析、DM足患病率及DM并发症发生状况比较,对ABI相关危险因素logistic回归性分析。结果：1268例患者中,ABI异常率为11.94%,A组占88.06%（236/268）,C组占10.07%,B组占1.86%;在≥60岁患者中,ABI异常率为8.58%,其中C组、B组所占比例均高于〈60岁者（P〈0.05）。2C组、B组DM足患病率较A组高（P〈0.05）。3ABI与舒张压、体重指数、甘油三酯、糖化血红蛋白、总胆固醇呈正比,与病程、年龄、24h尿微量白蛋白、血尿酸呈负比。结论：老年2型DM者具有较高的ABI异常率,ABI与DM足具有相关性,ABI异常升高或下降在DM足早期预测中具有积极的应用意义。
Objective： To investigate the ankle brachial index （ABI） of inpatients with diabetes mellitus （DM） in patients with diabetic foot prevention function.Methods：A retrospective analysis of the clinical data of patients with type 2 diabetes of 268 patients in our hospital in ABI, the patients were divided into 3 groups, group A （normal group）： 236 cases, ABI0.9 ~ 1.3; group B （increased group）： 5 cases, ABI = 1.3; group C （reducing group）： 27 example, ABI〈0.9; for each group of patients the clinical characteristics, the abnormal rate of ABI analysis, the prevalence of diabetic foot and diabetic complication status comparison, regression analysis on the risk factors related to logistic ABI.Results：In 268 patients, the abnormal rate of ABI was 11.94%, A group accounted for 88.06%（236/268）, C group, B group accounted for 10.07%, accounted for 1.86%; in more than 60 years old patients, the abnormal rate of ABI was 8.58%, the C group and B group were higher than the proportion of〈60 years of age （P〈0.05）. In C group, B group, the prevalence rate of diabetes foot higher than group A （P〈0.05）. The ABI and the diastolic blood pressure, body mass index, triglyceride and glycosylated hemoglobin, total cholesterol is proportional to, and course of disease, age, 24h urinary albumin, serum uric acid than a negative.Conclusion： The elderly patients with type 2 diabetes have higher abnormal rate of ABI, with the correlation between ABI and diabetic foot, abnormal rising of ABI or it is positively significant decreased in the diabetic foot in early prediction.
