摘要
目的:探讨三维超声(3D-US)和核磁共振成像(MRI)测量先天性膈疝胎儿对侧肺容积的相关性。方法:2010年1月-2013年2月在该院进行产前检查的55例胎儿,发现先天性膈疝,未合并其他畸形,经随访或尸检得到证实,染色体核型均为正常。24 h内行MRI检查,然后分别采用三维超声自动测量技术(VOCAL)和MRI T2加权多平面测量技术测定对侧肺容积,最后用一元回归分析确定两种方法的相关性。结果:55例胎儿中,左膈疝49例,右膈疝6例,平均孕周29周(范围18-39周)。评估对侧肺容积,3D-US和MRI则有着明显相关性(相关系数r=0.944,P〈0.001)。尽管3D-US测量膈疝胎儿肺容积的平均值要比MRI测量的平均值低22.9%,但是应用两种方法 (实测肺容积与预期肺容积比值:3D-US,0.47;MRI,0.51)无统计学差异。结论:对于单纯膈疝胎儿,3D-US可以准确测量胎儿对侧肺容积。
Objective: To explore the correlation between three- dimensional ultrasound( 3D- US) and MRI in measuring contralateral lung volume of fetuses with congenital diaphragmatic hernia( CDH). Methods: From January 2010 to February 2013,55 fetuses with isolated CDH were selected from the hospital after prenatal examination,all the fetuses were diagnosed definitely by follow- up or autopsy,chromosomal karyotypes were normal. MRI was performed with 24 hours after birth,three- dimensional ultrasonic automatic measurement technology and T2- weighted MRI were used to measure contralateral lung volume,the correlation was determined by monadic regressive analysis. Results: Among 55 fetuses,49 fetuses were diagnosed as left CDH and the other 6 fetuses were diagnosed as right CDH; the gestational weeks ranged from 18 to 39 weeks,29 weeks on average. Contralateral lung volume was assessed,there was a significant correlation between 3D- US and MRI( r = 0. 944,P 〈0. 001). Although the average value of contralateral lung volume measured by 3D- US was lower than that measured by MRI by 22. 9%,but there was no statistically significant difference between the two methods( the ratio of actual contralateral lung volume to expected contralateral lung volume: 3D- US: 0. 47; MRI: 0. 51). Conclusion: For the fetuses with isolated CDH,3D- US can measure contralateral lung volume accurately.
