5-氨基乙酰丙酸介导光动力治疗甲状旁腺功能亢进的前期研究

摘要 目的研究5-氨基乙酰丙酸(5-aminolevulinic acid,5-ALA)介导光动力治疗(photodynamic therapy,PDT)对正常大鼠甲状旁腺及其周围组织形态和功能的影响,探索采用该法治疗甲状旁腺功能亢进的可行性。方法 64只6~8周龄健康雌性SD大鼠,体质量150~200 g,按光敏剂剂量(0、100、300、500)mg/kg平均分为4个组,每组按光动力照射时间(0、10、15、20)min再分为4个亚组,共16个处理组(n=4),手术暴露甲状旁腺及其周围组织后行光动力照射,ELISA检测照射前后血清甲状旁腺激素(parathyroid hormone,PTH)、三碘甲状腺原氨酸(triiodothyronine,T3)、甲状腺素(thyroxine,T4)、促甲状腺素(thyroid-stimulating hormone,TSH)浓度,并观察照射前后各组织形态学上的变化。结果单独使用光敏剂或光照对甲状旁腺功能无影响,光动力治疗短时间内即可造成甲状旁腺功能的减低,而对甲状腺功能无显著影响,随着光敏剂浓度和光照时间的增加,其对甲状旁腺功能的破坏作用增强。当光敏剂浓度为500 mg/kg,光照时间为10、15、20 min时,血清PTH值分别为(75.24±6.06)ng/L、(47.23±5.18)ng/L、(39.10±4.03)ng/L,与对照组(0 mg/kg,0 min)(128.19±4.75)ng/L相比,具有显著性差异(P〈0.05)。组织学检查显示,经过光动力照射后的甲状旁腺组织与正常组比较,腺体包膜仍比较完整,其内纤维结缔组织增生明显,主细胞数明显减少,代之以大量空泡状的脂肪细胞,而甲状腺及其周围的肌肉组织在形态学上未见明显改变。结论以5-ALA为光敏剂介导的光动力疗法可对大鼠甲状旁腺组织及其功能产生特异性破坏作用。 Objective To determine the impact therapy (5-ALA PDT) on the morphology and function of Methods A total of 64 female SD rats, weighing 150 -200 of 5-aminolevulinic acid mediated photodynamic rat parathyroid glands and surrounding tissues. g, were randomly divided into 16 groups (n =4) according to the dose of photosensitizer (0, 100, 300 and 500 mg/kg) and irradiation time (0, 10, 15 and 20 min). The parathyroid gland and surrounding tissues were surgically exposed, followed by treatment with different doses of photosensitizer for different irradiation times. Before and after PDT, the serum levels of parathyroid hormone ( PTH), thyroxine ( T4), triiodothyronine ( T3 ) and thyroid-stimulating hormone (TSH) were determined by ELISA. Histological observation was carried out before and after treatment. Results 5- ALA PDT or irradiation alone had no significant change in parathyroid function. PDT induced parathyroid function reduction in a short time, but had no effect on thyroid function. With the increase of photosensitizer dose and irradiation time, the treatment showed significant effect on thyroid function. When the photosensitizer dose was 500 mg/kg and irradiation time was 10, 15 and 20 min, the serum level of PTH was 75.24 ±6.06, 47.23 ±5.18, and 39.10 ±4.03 ng/L, respectively, with significant difference as compared with control group (0 mg/kg and 0 min, 128.19 ±4.75 ng/L, P 〈0.05 ). After PDT, histological examination displayed integrated envelope of parathyroid, normal proliferation of fibrous connective tissue, increased number of vacuolated fat cells, and decreased number of gland cells compared with normal parathyroid, but no significant change was observed in the thyroid and surrounding muscles. Conclusion 5-ALA PDT specifically disrupts the morphology and function of parathyroid glands in rats.
