摘要
目的探讨老年偏瘫患者股骨粗隆间骨折采用亚洲型股骨近端抗旋髓内钉(PFNA-Ⅱ)内固定治疗的疗效。方法老年偏瘫股骨粗隆间骨折患者12例。国际内固定协会(AO/ASIF)分型:AO31.A1型3例,A2型7例,A3型2例。采用PFNA-Ⅱ内固定治疗,观察手术时间、术中出血量、临床愈合时间和术后并发症,评价髋关节功能恢复情况。结果 12例患者均顺利完成手术,手术时间35-75min,术中出血40-120ml。平均骨折愈合时间12周。髋关节X射线片显示骨折复位固定好,功能恢复满意。结论 PFNA-Ⅱ内固定治疗老年偏瘫患者股骨粗隆间骨折,固定稳定可靠,手术操作简单,创伤小。
Objective To explore the efficacy of appropriate specification proximal femoral nail antirotation-Ⅱ（PFNA-Ⅱ） internal fixation in treating elderly hemiplegia patients with intertrochanteric fractures.Methods PFNA-Ⅱinternal fixation was performed in 12 elderly patients with intertrochanteric fractures and hemiplegia,which were classified as AO/ASIF AO31.A1 in 3cases,A2 in 7cases,and A3 in 2cases.The operation time,blood loss,recovery time,postoperative complications and recovery of hip fuction were analyzed.Results The surgery was performed successfully in all cases with operation time of 35-75 min and blood loss of 40-120 ml.The healing time was 12 weeks in average.The internal fixation of fracture on X-ray film of the hip joint was good.The recovery of hip function was satisfactory in all cases.Conclusion Treating intertrochanteric fractures with PFNA-Ⅱinternal fixation has the advantages of stable fixation,simple procedure and less trauma in the elderly patients with hemiplegia.
