摘要
目的:对首诊癌症患者进行营养状况和生活质量调查并探讨二者的相关性。方法:选取2014年1月~12月在医科院肿瘤医院和北京同仁医院住院的首诊癌症患者549例,使用PG-SGA量表评估其营养状况,将其分为营养良好(A)组、可疑或中度营养不良(B)组和重度营养不良(C)组,调查并记录患者入院时的年龄、身高、体质量、1个月内体质量丢失百分比、体质量指数(BMI)、肱三头肌皮皱厚度(TSF)、上臂肌围(MAMC);血清白蛋白(Alb)、前白蛋白(PA)等指标;并使用SF-36健康调查量表评价患者的生活质量,分析营养状态与生活质量相关性。结果:①经PG-SGA量表评定,A组206例、B组257例、C组86例;②A组的体质量、BMI、TSF、MAMC均显著高于B组和C组(P〈0.05,P〈0.01);③A组的1个月内体质量丢失百分比、PG-SGA总分均低于B组和C组(P〈0.05,P〈0.01);④不同营养状态癌症患者生活质量各个维度得分差异显著(P〈0.05,P〈0.01),营养状况越好,生活质量越高。结论:癌症患者存在发生营养不良的风险且生活质量明显下降,营养状况与生活质量密切相关,提示提高患者营养状况可以改善生活质量。
Objective:To investigate the correlation between the nutritional status and the quality of life in patients with cancer.Methods:Five hundred and forty-nine cancer patients from Cancer Hospital Chinese Academy of Medical Science were recruited from January 2014 to December 2014.Nutritional status was evaluated by PG-SGA scale.The patients were classified as good nutrition PG-SGA(A)group,moderate PG-SGA(B)group and severe malnutrition PG-SGA(C)group.Age,height,bodyweight,percentage of weight loss in the previous month,body mass index(BMI),triceps skin fold thinness(TSF),arm muscle circumference(MAMC),serum albumin(Alb),and prealbumin(PA)were collected and Short Form of Health Survey(SF-36)was used to evaluate quality of life on admission.Results:There were 206,257 and 86 cases in group A,group B and group C,respectively.Bodyweight,BMI,TSF and MAMC in group A were higher than those in B group and C group;percentage of weight loss in the previous month and PG-SGA scores in group A were lower than those in group B and group C(P〈0.05,P〈0.01).In parallel,the scores of the quality of life were also significantly lower in Group C and that correlated to the nutritional status.Conclusion:The quality of life in patients with cancer was associated with the nutritional status.
