多层螺旋CT三维重建在眼部异物定位中的应用 被引量:10

Application of multi-slice spiral CT scan in the localization of ocular foreign body

摘要 目的评价多层螺旋cT在眼部异物定位中的应用效果。方法临床病例检测方法对照研究。2012年6月至2014年6月对在福建中医药大学附属人民医院就诊可疑眼部异物病例69例(69只眼),分别行多层螺旋CT三维重建和眼眶DR拍摄,计算两种方法异物的检出率;眼眶DR拍摄阳性者行角膜缝环-DR定位法,测量异物所在的点钟位,异物跟矢状面、水平面和角巩膜缘的距离。结果多层螺旋CT三维重建法检出异物26例(37.68%,26/69),眼眶正侧位DR拍摄法检出异物21例(30.43%,21/69)。两种方法异物检出率差异具有统计学意义(P=0.014)。DR拍摄法检出异物的点钟位,异物跟矢状面、水平面和角巩膜缘的距离与多层螺旋cT三维重建法相近,差异无统计学意义(点钟位t=1.103,P=0.272;异物与矢状面距离t=0.449,P=0.655;异物与水平面距离t=1.175,P=0.245;异物在角巩膜缘后距离t=1.575,P=O.117)。多层螺旋CT三维重建定位法的阳性率、灵敏度、正确指数、阴性预测值均高于角膜缝环-DR定位;多层螺旋CT三维重建定位法的假阴性率低于角膜缝环.DR定位。结论多层螺旋CT三维重建法异物检出率高,定位准确,可做为眼部异物定位的常规方法。 Objective To evaluate the application of multislice spiral CT scan in the localiza tion of ocular foreign body. Methods Sixtynine cases (69 eyes) that were suspicious of ocular for eign body were collected from June 2012 to June 2014 in our hospital. Those underwent threedi mensional reconstruction of multislice spiral CT scan and orbital digital radiography. The foreign body detection rates were calculated. The location (o'clock position) of foreign body and the distanc es of foreign body with sagittal, horizontal and limbus were measured. Results Twentysix cases were positive by threedimensional reconstruction of multislice spiral CT scan and the foreign body detection rate was 37.68% (26/69). While 21 cases were positive by orbital digital radiography and the foreign body detection rate was 30.43% (21/69). The difference was statistical significance (P = 0.014). The location (o'clock position) of foreign body and the distances of foreign body with sagit tal, horizontal and limbus measured by threedimensional reconstruction of multislice spiral CT scan were similar to those measured by orbital digital radiography. The differences were no statistical significance (the location of foreign body t =1.103, P =0.272; the sagittal distances of foreign body t =0.449, P =0.655; the horizontal distances of foreign body t =1.175, P =0.245; the distances be tween foreign body and limbus t =1.575, P =0.117). The positive rate, sensitivity, correct index and negative predictive value of multislice spiral CT scan were larger than those of orbital digital radiog raphy. The false negative rate of multislice spiral CT scan was smaller than that of orbital digital radiography. Conclusions Threedimensional reconstruction of multislice spiral CT scan owns high foreign body detection rate and accurate positioning. It can be used as a conventional method for oc ular foreign body localization.
