摘要
目的对比双锁定钢板与Y型钢板内固定治疗成人肱骨髁间骨折的临床效果。方法 64例肱骨髁间骨折患者分为Y型钢板组和双锁定钢板组,每组32例。Y型钢板组患者给予Y型钢板内固定治疗,双锁定钢板组患者给予双锁定钢板内固定治疗,比较2组患者的临床治疗效果。结果 2组患者手术时间、术中出血量、住院时间及骨折愈合时间比较差异均无统计学意义(P>0.05)。双锁定钢板组患者肘关节功能恢复优17例,良6例,可5例,差4例,优良率为87.50%;Y型钢板组患者肘关节功能恢复优13例,良6例,可3例,差10例,优良率为68.75%;双锁定钢板组患者肘关节功能恢复优良率高于Y型钢板组(P<0.05)。双锁定钢板组和Y型钢板组患者内固定失败率分别为6.25%(2/32)和15.63%(5/32),双锁定钢板组患者内固定失败率低于Y型钢板组(P<0.05)。结论双锁定钢板治疗肱骨髁间骨折可提供较好的稳定性和支撑力,有利于患者术后肘关节功能恢复。
Objective To compare the clinical effect of double locking plate fixation and Y-shape plate fixation in the treatment of adult humeral intercondylar fracture. Methods Sixty-four patients with adult humeral intercondylar fracture were divided into Y-shape plate group and double locking plate group,thirty-two patients in each group. The patients in Y-shape plate group were treated with Y-shape plate fixation,while the patients in double locking plate group were treated with double locking plate fixation. The clinical effect was compared between the two groups. Results There was no significant difference in the operation time,intraoperative bleeding volume,hospitalization time and fracture healing time between the two groups（ P 0. 05）. In double locking plate group,the recovery of joint function was excellent in seventeen cases,good in six cases,fair in five cases,and poor in four cases;the fineness rate was 87. 50%. In Y-shape plate group,the recovery of joint function was excellent in thirteen cases,good in six cases,fair in three cases,and poor in ten cases;the fineness rate was 68. 75%. The fineness rate in double locking plate group was higher than that in Y-shape plate group（ P 0. 05）. The failure rate of internal fixation in double locking plate group and Y-shape plate group was 6. 25%（2 /32） and 15. 63%（5 /32） respectively,the failure rate of internal fixation in double locking plate group was lower than that in Y-shape plate group（ P 0. 05）. Conclusion Double lock plate in the treatment of humeral intercondylar fracture can provide better stability and support,it is helpful for functional recovery of elbow joint after operation.
