Arthroscopic microfracture surgery combined with autologous bone marrow mesenchymal stem cells transplant in the treatment of knee cartilage defect

摘要 目的探讨关节镜下微骨折术联合自体骨髓间充质干细胞(BMSCs)移植治疗膝关节软骨缺损的方法和疗效。方法 60例膝关节软骨缺损患者随机分为对照组和治疗组,每组30例。对照组采用单纯关节镜下微骨折术治疗,治疗组采用关节镜下微骨折术联合自体骨髓间充质干细胞移植治疗。观察两组治疗效果。结果所有患者经过平均(16.4±3.3)(10~27)个月的随访,对照组VAS评分由术前平均(6.5±1.2)分降至(2.1±0.4)分,Lysholm膝关节功能评分由术前(43.2±8.7)分(37~56分)提高至术后(78.1±13.4)分(69~92分)。治疗组VAS评分由术前平均(6.7±1.3)分降至(2.0±0.3分),Lysholm膝关节功能评分由术前平均(42.6±8.9)分(34~58分)提高至术后(90.7±14.8)分(77~95)分。结论关节镜下微骨折术联合自体骨髓间充质干细胞移植创伤小、疗效可靠,是治疗膝关节软骨缺损的一种安全、有效的方法。 Objective To investigate the method and curative effect of arthroscopic microfracture surgery combined with autologous bone marrow mesenchymal stem cells(BMSCs) transplant in the treatment of knee cartilage defect. Methods A total of 60 patients with knee cartilage defect were randomly divided into control group and treatment group, with 30 cases in each group. The control group received arthroscopic microfracture surgery alone for treatment, and the treatment group received arthroscopic microfracture surgery combined with bone marrow autologous mesenchymal stem cells transplant. Curative effects were observed in the two groups. Results After receiving follow-up for average(16.4±3.3)(10~27) months, the VAS score in the control group reduced from(6.5±1.2) points to(2.1±0.4) points, and its Lysholm knee function score increased from(43.2±8.7) points(37~56 points) before operation to(78.1±13.4) points(69~92 points) after operation. The treatment group had decreased VAS scale score from(6.7±1.3) points to(2.0±0.3) points, and its Lysholm knee function score increased from(42.6±8.9) points(34~58 points) before operation to(90.7±14.8) points(77~95 points) after operation. The differences of indexes before and after operation between the two groups had statistical significance(P〈0.05). The treatment group had better Lysholm knee function score after operation than the control group, and the difference had statistical significance(P〈0.05). Conclusion Arthroscopic microfracture surgery combined with autologous mesenchymal stem cells transplant provides small trauma and reliable curative effect, and it is a safe and effective method for treating knee cartilage defect.
