摘要
目的探讨影响TLICS 4分胸腰椎爆裂骨折矫正度丢失的相关因素。方法以末次随访Cobb角丢失为结局指标,采用Logistic回归模型对影响胸腰椎骨折矫正度丢失的10个指标进行相关因素分析。结果本研究共纳入84例患者,随访时间18~31个月,平均21.8个月,术后41例Cobb角度丢失≤3°、43例≥4°,多因素Logistic回归分析结果显示,治疗方式、伤椎骨缺损程度及术后功能锻炼情况是影响TLICS 4分胸腰椎骨折矫正度丢失的主要因素（P〈0.05）。结论治疗方式、伤椎骨缺损程度及术后功能锻炼情况是TLICS 4分胸腰椎骨折术后Cobb角度丢失的独立因素。单纯后路短节段4钉固定治疗TLICS 4分胸腰椎骨折术后远期易出现矫正度丢失,合理的功能锻炼有助于维持术后矫正度。
Objective To explore the clinical relative factors of loss of correction in patients with TLICS 4 thoracolumbar burst fractures. Methods With loss of sagital Cobb angle as the primary outcome variable and non-conditional Logistic regression was used to analyse the correlation of the loss of Cobb angle with 10 factors. Results In this study, 84 patients meeting with the inclusion were collected. The follow-up time was 18 to 31 months（mean follow-up 21.8 months）. The result of the loss of sagital Cobb angle： angle loss was less than or equal to 3 degrees in 41 patients, equal or greater than 4 degrees in 43 patients. Monovariant and multivariant Logistic regression analysis demonstrated that the style of treatment, the degree of bone defect and situation of exercises were the influential factors of the loss of correction（P〈0.05）. Conclusion The style of treatment,the degree of bone defect and situation of exercises are the influential factors of the loss of correction in TLICS 4thoracolumbar burst fractures. There would be the loss of Cobb angle in TLICS 4 thoracolumbar fractures treated with short segment pedicle instrumentation. Reasonable exercises could maintain sagital Cobb angle.
