摘要
目的:探讨经腹部超声(TAS)联合经阴道超声(TVS)在宫外孕中的诊断价值。方法:回顾性分析2013年1月-2014年7月72例宫外孕患者的临床资料,所有患者均经TAS和TVS检查。分析其二维超声分型表现,比较TAS、TVS和TAS+TVS的诊断准确率和检出率。结果:72例宫外孕患者的二维超声可见26例未破裂型(附件区见类妊娠囊的环状高回声结构,内为小液性暗区)、35例流产型(附件区见边界不清的肿块,内部呈不均质高回声和液性暗区)、5例破裂型(宫旁见混合回声包块,内部回声杂乱)、1例宫角型(右宫角部见类妊娠囊的环状高回声结构,内为小液性暗区)和1例宫颈型(宫颈管内见高回声团块)。TAS、TVS和TAS+TVS诊断准确率分别为56.94%、90.28%、98.61%,差异比较有统计学意义(P<0.05)。TVS、TAS+TVS的附件区包块、卵黄囊、胎芽和胎心检出率明显高于TAS(P<0.05),且TVS检出率显著低于TAS+TVS(P<0.05)。结论:宫外孕患者的超声检查有其独特的分型表现,采用TAS+TVS检查可提高诊断准确率和检出率。
Objective：To explore the diagnostic value of abdominal ultrasound（TAS）combined with transvaginal ultrasound（TVS）on ectopic pregnancy.Methods：A retrospective analysis of 72 patients with ectopic pregnancy was selected from January 2013 to July 2014,all patients were confirmed by TAS and TVS.The performance of two-dimensional ultrasound was analysised,and diagnostic accuracy and detection rate of TAS,TVS and TAS＋TVS were compared.Results：In 72 cases of ectopic pregnancy,26 cases about the performance of two-dimensional ultrasound were unruptured（annex area gestational sac class ring hyperechoic structures within a small liquid dark area）,35 cases were miscarriage（annex area boundary was unclear mass with internal hyperechoic and liquid dark area）,5cases was rupture（hybrid echo uterine mass with internal echo clutter）,1case was uterine horn model（right cornual gestational sac class hyperechoic ring structure within a small liquid dark area）and 1case was cervical type（hyperechoic mass of the cervix）.There were statistically significant difference in the diagnostic accuracy rates of TAS（56.94%）,TVS（90.28%）and TAS＋TVS（98.61%）（P〈0.05）.The detection rates of accessories area mass,yolk sac,fetal bud and fetal heart by TVS or TAS＋TVS were significantly higher than those by TAS（P〈0.05）,which by TVS were significantly lower than those by TAS＋TVS（P〈0.05）.Conclusion：There were unique ultrasonography performance in patients with ectopic pregnancy,TAS＋TVS could improve diagnostic accuracy and detection rate.
