摘要
目的:建立"驾照式"合理用药记分卡管理体系,提高临床合理用药水平。方法:借鉴交通法规管理模式,按照管理模型化的思路,从管理领域、业务流程、业务规则与控制、角色和职能4个方面,统筹和设计"驾照式"合理用药记分卡管理,强化合理用药管理;比较管理前(2013年1-8月)、后(2014年1-8月)相关指标的统计结果以评价管理成效。结果:与管理前比较,实施管理后临床药师受邀参加各类临床会诊的次数从390次上升至719次,药师与医师互动增加,医师合理用药意识增强;对不合理用药干预,医师接受率从56.21%增加至85.53%;月均门诊处方和住院医嘱的不合理用药率均明显下降;实施管理后的2014年1-8月,门诊处方和住院医嘱的各月不合理率也呈下降趋势。结论:我院实施"驾照式"合理用药记分卡管理后,增强了临床医师合理用药意识,提升了对不合理用药的干预效能,提高了临床合理用药整体水平。
OBJECTIVE：To establish a management system with driver license-like scorecards for rational drug use to improve the management level of rational drug use. METHODS：By reference to the administration mode under the traffic laws,based on the management model,and from 4 aspects including management domain,business process,business rules control and roles functions,the overall plan and design were made for the management with driver license-like scorecards for rational drug use,and the management of rational drug use was strengthened. Statistical results of corresponding indexes before and after the management（from Jan. to Aug. 2013 and from Jan. to Aug. 2014,respectively）were compared to evaluate the effects of the management. RESULTS：Compared with before management,the number of various clinical consultations in which the clinical pharmacist was invited to participate was increased from 390 to 719. Pharmacists had more interactions with doctors. Doctors＇ awareness of rational drug use was increased. The rate of acceptance by doctors of intervention in irrational drug use was increased from 56.21% to85.53%. The average rate of monthly irrational drug use as shown in outpatient prescriptions and the doctor＇s orders for inpatients were decreased significantly,and there also was a downward trend in the rate of monthly irrational drug use in outpatient prescriptions and the doctor＇s orders for inpatients from Jan. to Aug. 2014. CONCLUSIONS：The management with driver license-like scorecards for rational drug use has increased clinicians＇ awareness of rational drug use,improved the effects of intervention in irrational drug use and promoted rational clinical drug use.
