摘要
目的探讨椎弓根钉棒系统内固定治疗伴神经症状腰椎骨折的疗效。方法对2011-02—2013-02在我院治疗且符合条件的64例腰椎骨折病例进行回顾性分析,按照病情将患者分为2组,其中陈旧性骨折组36例,新发骨折组28例。术后比较患者术前与术后的伤椎高度、Cobb角、水平位移、受压面积、矢状径,并进行神经系统评价。结果 2组患者的影像学指标组内比较时,手术后患者的伤椎高度、Cobb角、水平位移、受压面积、矢状径均较术前明显改善,差异均有统计学意义(P<0.05)。经1a的康复锻炼后,2组患者的ASIA损伤分级较术前有明显提高,差异均有统计学意义(P<0.05)。此外,在ASIA损伤分级的D、E级中新发骨折组与陈旧骨折组差异也有统计学意义(P<0.05)。结论在治疗腰椎骨折早期伴神经症状的患者中运用椎弓根钉棒系统内固定,能加快受损神经功能的恢复与预防迟发性神经损伤,具有更好的远期效果。
Objective To investigate the curative effect symptoms of patients with lumbar vertebra fracture. Methods of pedicle screw fixation systems on treating the neurological 64 with lumbar vertebra fracture in our hospital from February 2011 to February 2013 were divided into old fracture group (n= 36) and new fracture group (n= 28) according to the illness length. The injured vertebral height, Cobb angle, horizontal displacement, pressure area, sagittal diameter, and neurological function of two groups were compared before and after operation. Results The injured vertebral height, Cobb angle, horizontal displacement, pressure area and sagittal diameter of two groups before operation were better than these after operation. The A- SIA impairment of two groups after operation was significantly improved compared with that before operation. The Grade D and E of ASIS impairment in new fracture group were better than these in old fracture group. Conclusion Pedicle screw fixation systems has better curative effect on treating the neurological symptoms of patients with lumbar vertebra fracture in the early lumbar fractures, and can accelerate the damaged nerve function recovery and prevention the fate-onset nerve damage, which is worthy of application.
