Expression and prognostic significance of vascular endothelial growth factor in triple negative breast cancer

摘要 目的探讨血管内皮生长因子(VEGF)在三阴性乳腺癌(TNBC)中的表达及其预后价值。方法选择2000年6月-2014年10月中国石油天然气集团公司中心医院病理科的TNBC组织石蜡标本(TNBC组)和非三阴性乳腺癌组织石蜡标本(非TNBC组)为研究对象,各50例,另选择正常乳腺组织30例为正常对照组。采用SP免疫组织化学法检测VEGF蛋白在三组中的表达,并分析VEGF的表达与TNBC临床病理因素的关系。结果 VEGF在TNBC组、非TNBC组及正常乳腺组织中的阳性表达率分别为88.0%(44/50)、50.0%(25/50)、26.7%(8/30),TNBC组和非TNBC组中VEGF阳性表达率均明显高于正常对照组,而TNBC组中VEGF阳性表达率明显高于非TNBC组(χ2=8.17,P<0.05)。TNM分期(χ2=8.04,P<0.05)、组织学分级(χ2=7.39,P<0.05)、腋窝淋巴结转移(χ2=6.18,P<0.05)与VEGF表达的相关性差异有统计学意义。Logistic回归多因素分析结果显示腋窝淋巴结转移、TNM分期是VEGF表达的影响因素。结论 VEGF过度表达可能在TNBC的恶变、进展等过程中起更重要的作用,且其表达与TNBC的淋巴结转移、侵袭性强和容易远处转移的生物学行为密切相关。 Objective To explore the expression and prognostic value of vascular endothelial growth factor( VEGF) in triple negative breast cancer( TNBC). Methods We collected 50 tumor specimens from TNBC patients and 50 tissue specimens from non-TNBC patients; meanwhile,30 normal breast tissue specimens were collected as normal control group. SP immunohistochemical staining was used to determine the expression of VEGF. The relationship between VEGF expression and TNBC clinicopathological factors was analyzed. Results The positive expression rates of VEGF in TNBC group,non-TNBC group and normal control group were 88. 0 %( 44 /50),50. 0 %( 25 /50) and 26. 7 %( 8 /30),respectively. The positive expression rates of VEGF in TNBC group and non-TNBC group were both significantly higher than that in normal control group,while the positive expression rate of VEGF was significantly higher in TNBC group than in non-TNBC group( χ^2= 8. 17,P〈0. 05). The expression of VEGF in TNBC specimens were significantly associated with TNM stage( χ^2= 8. 04,P〈0. 05),histological grade( χ^2= 7. 39,P〈0. 05) and axillary lymph node metastasis( χ^2= 6. 18,P〈0. 05). The results of multifactor Logistic regression analysis showed that axillary lymph node metastasis and TNM stage were the factors influencing VEGF expression. Conclusions VEGF overexpression may contribute to the cancerization and development of TNBC;moreover,its expression is closely related to the biological behavior of TNBC,such as lymph node metastasis,strong invasion and easy to distant metastasis.
