摘要 目的探讨血红素加氧酶-1(HO-1)在糖尿病肾病的诊断价值,并对其临床诊断进行评价。方法建立模型大鼠,运用酶组织化学,灌流固定,冰冻切片,光镜下观察HO-1在正常大鼠肾脏及模型大鼠肾脏中的分布,对正常大鼠肾及模型组大鼠肾中HO-1存在部位进行定位。分别利用酶联免疫吸附试验(ELISA)和速率法测定健康对照组和糖尿病肾病组的血清、尿HO-1和尿肌酐(UCr)值。结果在糖尿病肾病模型中,HO-1主要在肾小球内表达。健康对照组、糖尿病肾病组、缓解组血清HO-1的值分别为28.21、43.25、30.01mmol/L。它们的尿HO-1/尿Cr比值分别为0.43、0.90、0.45。糖尿病肾病组的血清HO-1与尿HO-1/尿Cr分别明显高于健康对照组(P<0.05)。血清HO-1与尿HO-1/尿Cr诊断糖尿病肾病的敏感度分别为93%、95%,特异性都为85%。结论 HO-1定位精确、图像清晰,为HO-1的临床意义提供了客观依据。糖尿病肾病中HO-1明显升高,血清HO-1与尿HO-1/尿Cr对糖尿病肾病有较高诊断价值。 【Objective】To discuss the diagnostic value of heme oxygenase 1 in diabetic nephropathy, and to evaluate the clinical diagnosis.【Methods】Diabetic nephropathy model was established in rat. The tissues were perfused and fixed using enzyme histochemistry, the frozen sections were made. HO-1 distribution in the kidneys of the normal rats and the model rats was observed under light microscope and the site of heme oxygenase 1(HO-1) in the normal rat kidney and the model rat kidney was located. Serum and urinary HO-1 and urinary creatinine(UCr) values were determined by enzyme-linked immunosorbent assay(ELISA) and rate method respectively in the healthy control group and the diabetic nephropathy group. 【Results】In the diabetic nephropathy model, HO-1 expression was mainly in the glomeruli. In the healthy control group, the diabetic nephropathy and the ease group, HO-1 serum values were 28.21 mmol/l, 43.25 mmol/l, 30.01 mmol/l. Their urinary HO-1/Cr ratios were 0.43, 0.90 and 0.45. HO-1 in serum and urinary HO-1/Cr of the diabetic nephropathy group were significantly higher than those of the healthy control group(P〈0.05). The sensitivity of serum HO-1 and urinary HO-1/Cr for diabetic nephropathy diagnosis was 93% and 95%, and the specificity of both was 85%.【Conclusions】HO-1 can provide an objective basis for the clinical diagnosis of diabetic nephropathy with its accurate positioning and clear image. HO-1 is significantly increased in diabetic nephropathy, serum HO-1 and urinary HO-1/Cr are of high value in the diabetic nephropathy diagnosis.
