摘要
目的探讨65岁以上痔患者的微创手术治疗无痛病房管理效果。方法回顾性分析2007年7月至2012年7月手术治疗的65岁以上痔患者的106例临床资料。结果 106例中100%有效,出血症状和痔脱出症状均消失。11例肛缘水肿,17例排尿困难,5例感觉中度疼痛,101例感觉轻度疼痛。术后随访12~24个月,有2例少许出血,经保守治疗出血停止,无大便失禁、肛门狭窄等并发症。结论Ⅲ、Ⅳ期内痔、混合痔症状明显的患者应手术治疗,运用无痛病房的概念,进行合理的围术期处理和麻醉、手术方式的选择以及疼痛干预是取得痔手术无痛效果的关键。
Objective To investigate the effect of painless-ward management of patients over 65 years with microinvasive operation of hemorrhoids.Methods A retrospective study of 106 cases of patients with hemorrhoids operation over 65 years from July 2007 to July 2012 was analyzed.Results One hundred percent efficiency was found in 106 cases,and bleeding and hemorrhoid prolapse disappeared.The anal margin edema was observed in 11 cases,dysuria in 17 cases,moderate pain in 5cases and mild pain in 101 cases.Patients were followed up for 12 months to 24 months.Slightly bleeding was observed in 2cases,and the bleeding stopped after conservative treatment,no complications as encopresis,anal stenosis was observed.Conclusion Patients with phase Ⅲ or Phase Ⅳ hemorrhoids or obvious mixed hemorrhoids should accept operation.The key to reach the painless postoperative effect with the concept of painless-ward should be reasonable perioperative management,anesthesia choice,operation choice and pain intervention.
