摘要 目的探讨尿可溶性Fas(s Fas)和血管内皮生长因子(VEGF)在膀胱癌患者中表达的意义及相关性。方法采用病例对照研究方法,随机抽取膀胱癌患者和对照者各82例,采用酶联免疫分析法(ELISA)测定标本尿s Fas水平,采用免疫组织化学SP法检测膀胱癌标本中VEGF的表达。结果尿s Fas在膀胱癌病例组为(168.0±84.6)ng/ml,对照组为(56.2±37.0)ng/ml,差异有统计学意义(P<0.01)。尿s Fas随着病理分期的增加而增加(P<0.01)。VEGF在膀胱癌病例组的表达阳性率为74.4%(61/82),与膀胱癌病理分期、组织学分级呈正相关(P<0.01)。与VEGF阴性表达组比较,VEGF阳性表达组的尿s Fas浓度增高,并且尿s Fas浓度随着VEGF表达的增加而增高,差异有统计学意义(P<0.01)。结论 s Fas和VEGF可能在膀胱癌的发生、发展中发挥着协同作用,检测尿s Fas和VEGF有利于判断膀胱癌的进展和预后。 【Objective】 To investigate the significance of urine soluble Fas(s Fas) and vascular endothelial growth factor(VEGF) expression in bladder urothelial carcinoma(UC) and their correlation. 【Methods】 The case-control study was carried out. There were 82 cases with UC and the same number of the controls from the First Hospital of Shijiazhuang City and the Affiliated Hospital of Chengde Medical College at random. The level of s Fas was measured in the urine by enzyme-linked immunosorbent assay(ELISA) and the expression of VEGF protein of UC surgical specimens was screened by immunohistochemical SP method. 【Results】 s Fas levels were significantly higher in the urine of the patients with UC than in those without UC(168.0 ± 84.6)ng/ml vs(56.2 ± 37.0) ng/ml; P 〈0.01). The level was significantly higher in the cases with a higher stage than in those with a lower stage(P 〈0.01). The positive rate of VEGF expression was 74.4%(61 / 82) in the bladder urothelial carcinoma tissues. There was a positive correlation between the expression of VEGF protein and the pathological stage or grades in the UC tissues(each P 〈0.01). Compared with the negative group, urinary s Fas in the positive group of VEGF expression was higher(P 〈0.01). The level of s FAS was elevated with the increase of VEGF expression, and the difference was statistically significant(P 〈0.01). 【Conclusion】 Soluble Fas and VEGF may play synergic roles in the occurrence and development of bladder urothelial cancer. Effective detection of urine s Fas and VEGF protein may contribute to the evaluation of progress and prognosis in UC.
