摘要
目的探讨青年脑梗死(CI)患者血浆白蛋白(ALB)及纤维蛋白原(FIB)水平与其神经功能缺损严重程度的关系。方法选取2012年6月至2013年6月我院神经内科住院CI青年患者98例(观察组)和同期年龄低于45岁的非CI健康体检者30例(对照组)。统计分析两组ALB、FIB水平和神经功能缺损评分(NHISS),采用Pearson相关性分析法分析青年CI患者ALB及FIB水平与其神经功能缺损严重程度的关系,采用ROC曲线分析青年CI患者ALB和FIB水平对其神经功能缺损严重程度的预测作用。结果观察组治疗前的ALB、FI B水平及NHISS得分均高于对照组,治疗3天、1周、1个月、3个月和6个月的ALB、FI B水平和NHISS得分与治疗前比较,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,ALB及FIB水平均与其神经功能缺损严重程度呈显著正相关(r=0.802和0.814,P<0.01);ALB和FIB水平预测其神经功能缺损严重程度的ROC曲线下面积分别为0.765和0.748。结论青年脑梗死患者ALB和FIB水平均较正常人升高,且ALB和FIB水平均与其NHISS显著相关,青年脑梗死患者ALB和FIB水平的提高可在一定程度上预测其神经功能缺损的加重。
Objective To explore the relationship between the levels of plasma albumin( ALB) and fibrinogen( FIB) and the severity of nerve function defect in young patients with cerebral infarction( CI). Methods Ninety-eight young CI patients in our hospital from June,2012 to June,2013 were selected as observation group and 30 healthy persons under 45 years old were selected as control group. Plasma levels of ALB and FIB and neurologic impairment score( NHISS) before and after treatment were statistically analyzed.Pearson correlation analysis was used to analyze the relationship between the plasma levels of ALB / FIB and NHISS. ROC curve was also used to analyze of prediction value of plasma levels of ALB and FIB in neurologic impairment severity. Results Plasma levels of ALB and FIB and NHISS in the observation group before treatment were all higher than that in the control group. The indexes after 3days,1 week,1 month,3 months and 6 months of treatment were significantly different from before treatment( P〈0. 05). Pearson correlation analysis showed that plasma levels of ALB and FIB and NHISS were significantly positively correlated( r = 0. 802 and 0. 814,P〈0. 01). Area under ROC curves of plasma levels of ALB and FIB were 0. 765 and 0. 748,respectively. Conclusion Plasma levels of ALB and FIB are increased in young IC patients. Plasma levels of ALB and FIB are significantly associated with NHISS. Plasma levels of ALB and FIB can be used to predict neurologic impairment severity in these patients.
