摘要
目的:了解某医院近5年出院死亡病例的分布特征和变化趋势,为疾病的重点防治和医院医疗资源的优化配置提供参考依据。方法:对出院死亡病例整理后,按照性别、年龄、疾病种类进行统计分析。结果:该院住院病死率呈逐年下降趋势(趋势χ2=17.269,P<0.001);男性住院病死率(2.3%)高于女性(1.3%)(χ2=112.103,P<0.001);随着年龄的增长,病死率呈递增趋势,60岁以上的死亡人数占总死亡人数72.6%;按疾病分类排在死因前三位的依次是恶性肿瘤(48.8%)、循环系统疾病(18.4%)、呼吸系统疾病(10.6%);按病种排在死因前五位的依次为肺癌(14.5%)、肝癌(6.7%)、重症肺炎(3.6%)、脑出血(3.3%)和慢性肾功能衰竭(3.1%)。结论:卫生机构应加强健康宣传教育,提高群众自我保健意识,改变不良生活方式,政府部门要加强环境保护,医院可针对恶性肿瘤、循环系统疾病和呼吸系统疾病等重点疾病采取有效措施,开展多样化多层次的健康管理工作,降低病死率。
Objective To find out the distribution characteristics and change trends of discharged death cases from a hospital nearly five years,and provide a basis for reasonable allocation of disease prevention and health care resources. Method Sex,age and types of diseases of all death cases were analyzed. Results The fatality rate was decreasing year by year（ trend χ^2= 17. 269,P 0. 001）; Male＇s hospital mortality rate（ 2. 3%） was higher than female＇s（ 1. 3%）（ χ^2= 112. 103,P 0. 001）; With the growth of the age,the mortality rate increased,the death toll over 60 years represented 72. 6% of all deaths; The top three causes of death were malignant tumor（ 48. 8%）,circulatory system diseases（ 18. 4%）,and respiratory system diseases（ 10. 6%）; Ranked in the top five causes of death were followed by lung cancer（ 14. 5%）,liver（ 6. 7%）,pneumonia（ 3. 6%）,cerebral hemorrhage（ 3. 3%） and chronic renal failure（ 3. 1%）. Conclusion It suggests that health institutions should strengthen propaganda,improve people＇ s disease prevention consciousness,change their bad life habits,the government should protect the environment,and improve people＇s health level. To reduce the fatality rate,our hospital could take effective measures and carry out diversified multi-level health management work for priority diseases.
