三维盆底超声对晚孕期女性盆膈裂孔形态观察 被引量:9

摘要 目的利用三维盆底超声观察晚孕期盆膈裂孔(levator hiatus,LH)的形态。方法选取2012年3月~2013年6月在新疆医科大学第五附属医院产科定期产前检查的初产妇40例,均为单胎妊娠,孕32~40周(晚孕组);同期选择符合条件的未育女性20例为对照组,利用三维超声对受检者进行探查,分别获得晚孕组及未育女性在静息状态、Valsalva状态、缩肛状态下的盆膈裂孔图像及盆膈裂孔的左右径(LHLR)、前后径(LHAP)、周长(LHC)及面积(LHA)的测量值,比较各组图像各指标之间的差异性及相关性。结果晚孕期女性盆膈裂孔的LHLR、LHAP、LHC、LHA均大于未育女性,各指标差异均有统计学意义。结论晚孕期女性盆膈裂孔较未育女性大,三维盆底超声可早期发现盆膈裂孔形态改变,对改善女性产后生活质量有重要意义。 Objective To observe the form of levator hiatus at late pregnancy by three-dimensional ultrasound.Methods 40 late pregnancy women treated at the Five Affiliated Hospital of Xinjiang Medical University during March 2012~June2013,with gestational age from 32 weeks to 40 weeks,were named test group,20 healthy nulliparas with matched age,height,body mass index were taken as control group during the same period.We measured the diameter of the levator hiatus(levator hiatal area,LHA),anteroposterior diameter of levator hiatus(levator hiatal anteroposterior diameter,LHAP),transverse diameter of levator hiatus(levator hiatal lateral diameter,LHLR)and circumference of levator hiatus(levator hiatal circle,LHC)by transperineal three-dimensional ultrasound at three different states(including rest,Valsalva action and levator ani muscle maximum contraction)and compared them.Results At rest,Valsalva movements and levator ani muscle contraction state,the measurement data of test group were greater than control group,there were significant differences between them(P〈0.05).Conclusion There are impacts of late pregnancy on levator hiatual morphous for women,the three-dimensional ultrasound is significant for improving the quality of life after women delivery.
