摘要
目的探讨慢性阻塞性肺疾病稳定期老年患者血清白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)的水平及与吸烟、体质量指数(BMI)的关系。方法选取2012年12月至2014年2月到甘肃省人民医院就诊的门诊或住院的慢性阻塞性肺疾病(COPD)稳定期的吸烟老年患者50例,已戒烟的老年COPD患者45例,已戒烟的健康体检者40名,检测3组血清IL-6、TNF-α、CRP浓度,并对上述炎症因子与吸烟COPD患者吸烟指数(SI)、BMI进行相关性分析。结果稳定期吸烟老年COPD组血清IL-6、TNF-α、CRP水平明显高于已戒烟COPD组[IL-6分别为(45.9±12.1)、(38.1±9.6)mg/L,TNF-α分别为(58.2±15.8)、(45.9±11.2)ng/L,CRP分别为(12.2±4.1)、(8.6±3.2)mg/L]和健康对照组[IL-6(17.0±9.9)mg/L、TNF-α(27.3±13.2)ng/L、CRP(6.3±5.2)mg/L],差异均有统计学意义(F值分别为84.934、57.224、23.023,P均〈0.01)。S11〉400者,血清IL-6、TNF-α、CRP水平明显高于SI〈400者[IL-6分别为(50.1±12.1)、(41.0±10.2)mg/L,TNF-α分别为(64.2±12.6)、(47.8±14.0)ng/L,CRP分别为(13.4±3.7)、(10.8±4.2)mg/L],差异有统计学意义(t值分别为2.845、4.343、2.347,P〈0.01或P〈0.05);且吸烟COPD组血清IL-6、TNF-α、CRP水平与患者SI呈正相关,相关系数分别为0.458(P〈0.01)、0.438(P〈0.01)、0.313(P〈0.05),血清IL-6、TNF-α水平与BMI呈负相关,相关系数分别为-0.358、-0.319(P均〈0.05)。结论吸烟引起老年稳定期COPD患者IL-6、TNF-α和CRP水平明显升高,可能在COPD的发病中起重要作用。且长期吸烟引起患者BMI下降,吸烟患者BMI下降与血清IL-6、TNF-α高表达相关。
Objective To investigate the relationship between serum interleukin 6 (IL-6), tumor necrosis factor α(TNF-α) and C reaction protein(CRP) levels and smoking and body mass index(BMI) in the elderly patients with stable chronic obstructive pulmonary disease (COPD). Methods Elderly participants including 50 cases smokers with stable COPD,45 cases ex-smokers with stable COPD and 40 cases healthy ex- smokers were recruited in this study, who were hospitalized in the People's Hospital of Gansu Province from Dec. 2012 to Feb. 2014. Serum IL-6, TNF-α and CRP levels were detected. Correlation analysis was performed between serum IL-6, TNF-α, CRP levels and smoking index( SI), BMI in COPD smokers. Results The levels of serum IL-6 ,TNF-oL and CRP in smoking group were (45.9 ±12. 1 ) mg/L, (58.2± 15.8) ng/L, ( 12. 2±4. 1 ) mg/L, significantly higher than those in stop-smoking group ( (38. 1±9. 6 ) mg/L, (45.9 ± 11.2 ) ng/L, (8. 6±3.2) mg/L respectively ), and healthy controls group ( ( 17.0 ± 9.9 ) mg/L, ( 27. 3±13.2 ) ng/L, ( 6. 3±5.2) mg/L) ,and the differences were significant( F = 84. 934,57. 224,23.023 ;P 〈 0. 01 ). In patients with smoking index≥400,the levels of serum IL-6,TNF-α and CRP were (50. 1 ± 12. 1 ) mg/L, (64. 2±12. 6) ng/L, ( 13.4 ± 3.7 ) mg/L, significantly higher than those in patients with SI 〈 400 group ( ( 41.0 ± 10. 2 ) mg/L, (47.8± 14. 0 ) ng/L, ( 10. 8± 4. 2 ) mg/L respectively), and the differences were significant ( t = 2. 845, 4. 343,2. 347;P 〈 0. 01 or P 〈 0. 05).
