摘要
[目的]观察针刺治疗结核性胸膜炎并发呃逆疗效。[方法]使用前瞻性设计,将55例门诊及住院患者针刺,取穴巨阙、中脘、内关双、合谷双、阴陵泉双、足三里双、三阴交双、太冲双、攒竹双、镇逆（攒竹双＋与攒竹二穴形成等边三角形之上角）、翳风双,取仰卧位,无菌针灸针（0.25×25mm、0.25×40mm）,先取双侧内关,直刺0.5寸,施平补平泻法;巨阙、中脘直刺2-2.5寸,行捻转补法;双侧合谷、太冲行提插泻法;双侧足三里、阴陵泉、三阴交直刺行补法;双侧攒竹,沿眶上切迹刺入0.5寸,与攒竹二穴形成等边三角形之上角向下直刺或斜刺0.5寸,施泻法,留针30min;起针后针刺双翳风,直刺0.5-1寸,提插捻转强刺激,同时做吞咽动作,施术1min,起针。连续治疗5d为1疗程。观测临床症状、呃逆、不良反应。治疗1疗程,判定疗效。[结果]痊愈40例,有效11例,无效4例,总有效率92.72%。[结论]针刺治疗结核性胸膜炎并发呃逆效果显著,值得推广。
[Objective] To observe the acupuncture treatment of tuberculous pleurisy complicated hiccup effect. [Methods] Using a prospective design,the 55 cases of outpatient and inpatient acupuncture,acupoints Juque,Zhongwan,the customs pairs,gu double yinlingquan double,double Zusanli,Sanyinjiao double,Taichong double,save bamboo double,town inverse（double ＋ formation Cuanzhu Cuanzhu two points of an equilateral triangle upper corner）,Yifeng double,supine position,sterile acupuncture needles,size 0.25 × 25 mm,0.25 × 40 mm,to take sides in the customs points,piercing the 0.5-inch,Shi reinforcing-reducing method; Juque,Zhongwan piercing 2 to 2.5 inches,the line twisting and fill method; bilateral gu,Taichong line interpolation mention reducing method; bilateral Zusanli yinlingquan Sanyinjiao piercing line completion method; bilateral Cuanzhu point along the supraorbital notch piercing 0.5 inch,0.5 inch perpendicularly or obliquely downward and upper corners of an equilateral triangle formation Cuanzhu two points,Shi reducing method,the needle 30min; after acupuncture needles double Yifeng,piercing the 0.5 to 1 inch,put plug Twist strong stimulation,while the swallow,the treatments 1min,the needles. 5d continuous treatment for a course of treatment. Observation of clinical symptoms,hiccups,adverse reactions. A course of treatment to determine efficacy. [Results] Cured 40 cases,effective in 11 cases,4 cases,the total efficiency of 92.72%. [Conclusion]Acupuncture treatment of tuberculous pleurisy complicated hiccup effect is remarkable,worthy of promotion.
