摘要
目的建立一套科学、实用、简便的连续性肾脏替代治疗(CRRT)容量状况评分表,为CRRT患者容量管理提供依据。方法通过循证查阅容量监测相关文献,初步拟定评价指标;采用德尔菲法对8名专家进行2轮咨询筛选指标,并基于头脑风暴法对无循证支持的指标进行量化、评分。结果 2轮问卷回收率均为87.5%,专家权威系数分别为0.89和0.90;CRRT容量状况评分表包含常规血流动力学监测、PiCCO容量相关指标、血管活性药物、临床症状4个维度、15个监测指标,并对每个指标进行量化和评分。结论 CRRT容量状况评分表具有较好的可靠性和权威性,可指导临床医务人员对CRRT患者进行容量管理。
Objective To establish a scientific,practical and simple rating table for the capacity status of patients undergoing continuous renal replacement therapy（CRRT）.Methods A preliminary scoring sheet was set up based on an evidence-based literature review.Two rounds of consultation were conducted among eight experts using the Delphi method,and brainstorming was used to tackle indicators without medical evidence to be based on.Results The response rates were 87.5% in two rounds of expert consultation,and expert authority coefficient was 0.89 and 0.90 respectively.The scoring table established consisted of four dimensions of routine hemodynamic monitoring,the related indexes of PiCCO capacity,vasoactive agents and clinical symptoms,totally 15 indicators.Conclusion The scoring sheet is of good reliability and authority,and can be used by medical staff to monitor the volume status of CRRT patients.
