摘要
目的探讨缺氧缺血性脑病(HIE)患儿外周血调节性T细胞(Treg)和辅助性T细胞17(Th17)及其细胞因子白细胞介素(IL)-10、IL-17、IL-23、转化生长因子-β(TGF-β)水平的变化。方法选取2012年1月至2014年1月在临颍县人民医院接受治疗的HIE患儿82例作为观察组,并选取40例健康新生儿作为对照组。抽取新生儿外周血,应用流式细胞仪检测全血中Treg和Th17的比例,采用酶联免疫吸附测定法检测血清中IL-10、IL-17、IL-23和TGF-β水平。结果轻度HIE组患儿外周血Treg水平与对照组比较差异无统计学意义(P>0.05),中度HIE组和重度HIE组患儿外周血Treg水平显著低于对照组(P<0.05);轻度HIE组患儿外周血Th17水平与对照组比较差异无统计学意义(P>0.05),中度HIE组和重度HIE组患儿外周血Th17水平显著高于对照组(P<0.05)。轻度HIE组患儿血清IL-10、TGF-β、IL-17和IL-23水平与对照组比较差异无统计学意义(P<0.05),中度HIE组和重度HIE组患儿血清IL-10和TGF-β水平显著低于对照组(P<0.05),而IL-17和IL-23水平则显著高于对照组(P<0.05)。结论 HIE患儿存在外周血Treg、Th17及其细胞因子表达的失衡,检测外周血Treg、Th17及其细胞因子水平对判断HIE病情进展、指导临床治疗有一定的临床意义。
Objective To investigate the changes of peripheral blood T regulatory cells(Treg),T helper cells 17(Th17) and their cytokines interleukin(IL)-10,IL-17,IL-23 and transforming growth factor-β(TGF-β) in newborns with hypoxic ischemic encephalopathy(HIE).Methods A total of 82 newborns with HIE in the People's Hospital of Linying County from January 2012 to January 2014 were selected as HIE group,and 40 healthy newborns were selected as control group.Peripheral blood of newborns was collected in the two groups.The Treg and Th17 cells in whole blood were detected by flow cytometer.The levels of serum IL-10,IL-17,IL-23 and TGF-β were detected by enzyme-linked immunosorbent assay method.Results There was no significant difference in peripheral blood Treg cells level between mild HIE group and control group(P 〈0.05).The Treg cells level of peripheral blood in moderate and severe HIE groups was significantly lower than that in control group(P 〈0.05).There was no significant difference in peripheral blood Th17 cells level between mild HIE group and control group(P 〉0.05).The Th17 cells level of peripheral blood in moderate and severe HIE groups was significantly higher than that in control group(P 〈0.05).There was no significant difference in the levels of serum IL-10,TGF-β,IL-17 and IL-23 between mild HIE group and control group(P 〈0.05).The levels of serum IL-10 and TGF-β in moderate and severe HIE groups were significantly lower than those in control group(P 〈0.05).The levels of serum IL-17 and IL-23 in moderate and severe HIE groups were significantly higher than those in control group(P 〈0.05).Conclusions The imbalance of Treg cells,Th17 cells and their cytokines exist in newborns with HIE.The detection of Treg cells,Th17 cells and their cytokines levels has certain clinical significance for judging HIE progression and guiding the clinical treatment.
