摘要
目的探讨血清同型半胱氨酸(HCY)、血浆内皮素-1(ET-1)、B型利钠肽(BNP)及D-二聚体(D-D)联合检测诊断急性脑梗死(ACI)的价值。方法选取36例ACI患者及30同期健康体检者,比较不同程度神经功能缺损急性脑梗死患者与健康对照组血HCY、ET-1、BNP及D-D的水平。结果急性脑梗死组患者血HCY、ET-1、BNP及D-D水平均显著高于健康对照组(P<0.05);HCY、ET-1、BNP及D-D单独检测脑梗死阳性率分别为47.2%、36.1%、33.3%及47.2%,明显低于4项联合检测的97.2%阳性率,差异有统计学意义(P<0.05);HCY及D-D水平随神经功能缺损程度的加重逐渐升高,差异有统计学意义(P<0.05)。结论血HCY、ET-1、BNP及D-D水平变化与急性脑梗死发生发展密切相关,联合检测有助于急性脑梗死的诊断。
Objective To explore the value of combined detection of serum homocysteine (HCY), plasma endothelin-1 (ET-1), B type natriuretic peptide (BNP) and D-dimer (D-D) in the diagnosis of patients with acute cerebral infarction (ACI). Methods A total of 36 ACI pa- tients and 30 healthy volunteers were selected, and serum HCY, ET-1, BNP and D-D levels were compared between patients with different degrees of ACI and control group. Results Serum HCY, ET-1, BNP and D-D levels in ACI group were significantly higher than those in the control group (P 〈 0.05). Positive rates of HCY, ET-1, BNP and D-D for detection of cerebral infarction were 47.2 %, 36.1%, 33.3 % and 47.2 % respectively, which were significantly lower than the 97.2 % of combined detection of four indexes (P 〈 0.05). HCY and D-D levels increased gradually with the impairment degree of neural function, and there were significant differences (P 〈 0.05). Conclusion Serum HCY, ET-I, BNP and D-D levels are closely related to the occurrence and de- velopment of acute cerebral infarction, and combined detection is helpful for diagnosis of acute cere- bral infarction.
