摘要
目的分析艾滋病抗病毒治疗终止的原因及其可能的影响因素。方法收集福建省2004年1月到2013年12月的艾滋病抗病毒治疗患者资料,共计2 639例,分析基线情况对终止治疗的影响。两组间计数资料采用χ2检验,等级资料比较采用Mann-Whitney U秩和检验和Pearson积差相关分析,多因素分析采用logistic回归分析。结果 2 639例艾滋病患者中,301例在开始抗病毒治疗后因各种原因终止治疗。艾滋病患者终止治疗的主要原因为死亡(188例,占62.46%),其次是失访(57例,占18.94%),最后是停药(56例,占18.60%)。年龄较大、开始治疗前有相关疾病及症状体征、抗体确证到启动治疗的时间间隔短和基线CD4+T细胞水平低增加患者终止治疗概率。结论加强依从性教育,在符合治疗标准后尽早开始治疗可以减少艾滋病抗病毒治疗患者终止治疗。
Objective To discuss the factors leading to the termination of antiretroviral therapy（ART）among AIDS patients in Fujian. Methods Data of 2639 AIDS patients receiving free ART were collected from January 2004 to December 2013 in Fujian to analyze the influence of baseline situation on the termination of ART.Chi-square test,Mann-Whitney U test,Pearson correlation and multivariate logistic method were used for data statistical analysis. Results 301 out of 2639 AIDS patients ended the therapy at the beginning of treatment mostly because of death,accounting for 62.46%,followed by failure of follow-up（18.94%）and drug withdrawal（18.60%）.Patients with older age,low baseline CD4＋T cells or with other health issues and symptoms before treatment and short time interval from antibody confirmation to the initial of treatment were more likely to terminate the ART. Conclusions The most common factors leading to the termination of ART for AIDS patients are death and failure to follow-up.Patients who meet the ART requirement should be treated as earlier as possible to avoid therapy termination.
