摘要
目的:探讨参附注射液对食道癌根治术所致肺损伤的保护作用。方法:60例限期行食道癌根治术患者,随机分为参附注射液组(n=30)和对照组(n=30)。于麻醉诱导后参附注射液组按1 m L/kg静脉给予参附注射液,对照组给予等容量的生理盐水。于单肺通气开始时(T0)、单肺通气1 h(T1)、恢复双肺通气3 h时(T2)各监测时间点抽取动脉血5 m L,用酶联免疫吸附法(ELISA)法测定血浆中白介素6(IL-6)、白介素8(IL-8)和肿瘤坏死因子α(TNF-α)的浓度。结果:两组患者T1、T2时点的血浆IL-6、IL-8和TNF-α的水平均较T0明显升高(P<0.05)。参附注射液组在T2时点的血浆IL-6、IL-8水平以及T1、T2时点的血浆TNF-α水平均明显低于对照组(P<0.05)。结论:参附注射液减轻了食道癌根治术单肺通气期间及其后血浆炎症因子IL-6、IL-8和TNF-α的水平,减轻了炎症反应。
Objective: To investigate the clinical effects of Shenfu injection(SF) on inflammatory response after esophagectomy.Methods: 60 Patients ready for esophagectomy were divided into two groups randomly, each group consisting of 30 cases. In Shenfu injection(SF) group, patients were given 1 mL/kg Shenfu injection after anesthesia induction. In control group, patients were given normal saline of equal volume after anesthesia induction. Plasma IL-6, IL-8 and TNF-α levels were measured at baseline(T0), 1 h after one lung ventilation(T1) and 3 h after lung reinflation(T2). Results: Compared with baseline level, plasma IL-6, IL-8 and TNF-α levels at T1, T2 point were both increased(P〈0.05). Compared with the control group, patients in SF group had reduced plasma IL-6, and IL-8 level at T2 point, and reduced TNF-α at both T1 and T2point(P〈0.05). Conclusion: Shenfu injection can reduce plasma IL-6, IL-8 and TNF-α levels after esophagectomy while minimizing inflammation.
