摘要
目的探讨立体定向手术治疗难治性抑郁症患者症状及睡眠质量的变化。方法采用立体定向手术治疗抑郁症患者61例,术前及术后3个月采用汉密尔顿抑郁量表(HAMD-17)和匹兹堡睡眠质量指数量表(pSQI)对患者进行评分,根据临床疗效评价标准和HAMD-17减分率来对患者的疗效进行评估,PSQI评分评估患者睡眠质量的改善情况。结果临床疗效评价结果:患者术后3个月临床恢复5例(8.19%),显著改善23例(37.70%),改善25例(40.98%),无效8例(13.13%),总有效率86.87%。HAMD-17减分率疗效评价结果:患者手术3个月后痊愈7例(11.48%),显著进步22例(36.07%),进步24例(39.34%),无效8例(13.11%),总有效率为86.89%。HAMD-17评分显示各个因子和总分与术前相比均显著降低(P<0.01)。PSQI评分各个因子和总分较术前均显著减少(P<0.01)。结论立体定向手术治疗难治性抑郁症可有效改善抑郁症状,同时全面提高睡眠质量。
Objective To investigate the changes of stereotactic operation in treating refractory symptoms and sleep quality of patients with depression.Methods Stereotactic operation for the treatment of depression were performed in 61 cases,Hamilton Depression Scale（HAMD-17） and Pittsburgh sleep quality index（PSQl） score were tested on these patients before operation and 3 months after operation,according to the clinical efficacy evaluation criteria and HAMD-17 score reduction rate to evaluate the curative effect,PSQ1 score was used to evaluate the situation,improvement of the quality of sleep.Results Clinical evaluation results：after 3 months,5 patients were clinical recovered（8.19%）,significantly improved in 23cases（37.70%）,improved in 25 cases（40.98%）,and invalid in 8 cases（13.13%）,the total efficiency were 86.87%.HAMD-17 reduction rate of curative effect evaluation results：in the 3 months after operation,7 cases were cured（11.48%）,22 cases with significant progress（36.07%）,progress in 24 cases（39.34%）,invalid in 8 cases（13.11%）,the total efficiency was 86.89%.HAMD-17 score reveal that all the factors and total score decreased significantly compared with the before operation（P〈0.01）.The PSQl score of each factor and total score were significantly lower than that before operation（P〈0.01）.Conclusion Stereotactic surgery in the treatment of refractory depression can effectively improve the symptoms of depression,and comprehensively improve the quality of sleep.
