高频超声应用在手部肌腱断裂缝合术中的作用 被引量:7

摘要 目的探讨高频超声在手部肌腱断裂缝合术中的应用价值。方法选择2009年3月~2013年6月在东阳市横店集团医院手外科就诊并术前进行超声定位的98例手部肌腱断裂患者为实验组,2014年1~6月就诊未行超声定位的48例患者为对照组,分别记录两组患者手术时间,术后高频超声探查肌腱修复效果和评估术后手指康复情况,并进行统计分析。结果两组最终完成全部超声监测检查的为实验组76例、对照组40例。实验组平均手术时间明显少于对照组[(30±20)min比(60±30)min],差异有高度统计学意义(P〈0.01);实验组术后腱性连接率高于对照组[(72.4%,55/76)比(52.5%,21/40)],实验组瘢痕形成率低于对照组[(27.6%,21/76)比(47.5%,19/40)],差异均有统计学意义(P〈0.05);实验组TAM手部功能康复评估良好率高于对照组[(86.8%,66/76)比(67.5%,27/40)],差异有统计学意义(P〈0.05)。结论采用高频超声可明显降低手术时间和术后瘢痕连接,术后高频超声持续监测肌腱修复水平可明显提高术后手部功能康复。 Objective To explore the role of high frequency ultrasound application in hand tendon rupture suture.Methods From March 2009 to June 2013 in Hengdian Group Hospital of Dongyang City, 98 patients diagnosed as hand tendon rupture and using preoperative ultrasound location were selected as experiment group, 48 patients with hand tendon rupture without underwent preoperative ultrasound location from January to June 2014 were selected as control group. Operating time was recorded in two groups, high frequency ultrasound was used to explore the effect of tendon repair and evaluate postoperative hand functional recovery, the results of two groups were statistical analyzed. Results In experiment group, 76 cases accomplished all ultrasoand lation examinatious, and 40 cases accomplished those in coutrol group. Average operation time in experiment group was significantly less than that in control group [(30±20) min vs(60 ±30) min], the difference was statistically significant(P〈 0.05); tendon connection rate in experiment group was higher than that in control group [(72.4%, 55/76) vs(52.5%, 21/40)], scar rate in experiment group was lower than that in control group [(27.6%, 21/76) vs(47.5%, 19/40)], the differences were statistically significant(P〈 0.05); TAM hand functional recovery assessment well rate in in experiment group was higher than that in control group [(86.8%,66/76) vs(67.5%,27/40)], the difference was statistically significant(P〈 0.05). Conclusion Preoperative using high frequency ultrasound can significantly decrease the operating time and preoperative scar connection, and preoperative high-frequency ultrasonic continuous monitoring the hand tendon repair can significantly improve the recovery of hand function.
