摘要
目的探讨全髋关节置换术和半髋关节置换术治疗老年股骨颈骨折的效果对比情况。方法选择沈阳市骨科医院骨外科2013年1月～2014年1月收治的120例老年股骨颈骨折患者的临床资料,依据采取的置换术方法不同进行分组,半髋组60例和全髋组60例。观察两组术后功能评分、Barthel指数评分、术后股骨头缺血坏死、股骨颈缩短、感染情况。结果全髋组术后功能评分优良率（100.0%,60/60）明显优于半髋组（80.0%,48/60）,差异有统计学意义（χ2=22.22,P〈0.05）;全髋组患者Barthel指数评分情况优良率（100.0%,60/60）明显优于半髋组（73.3%,44/60）,差异有统计学意义（χ2=30.81,P〈0.05）;全髋组患者术后股骨头缺血坏死、股骨颈缩短、感染发生率均低于半髋组（0.0%比10.0%、0.0%比6.7%、0.0%比13.3%）,差异有统计学意义（χ2=10.53、6.93、14.25,P〈0.05）。结论全髋关节置换术治疗老年股骨颈骨折患者临床效果明显,预后良好,值得临床推广应用。
Objective To approach curative effect of treatment agedness femur neck fracture by total hip replacement surgery and half a hip replacement surgery. Methods From January 2013 to January 2014, clinical data of 120 patients with femur neck fracture in Orthopedic Hospital of Shenyang City, according to bone surgery method, which were divided into half hip group（60 cases） and total hip group（60 cases）. The postoperative functional score, Barthel index score,postoperative ischemic necrosis of femoral head, femoral neck to shorten, infection of two groups were detected. Results The postoperative functional score good rate of total hip group（100.0%, 60/60） was better than that of half hip group（80.0%, 48/60）, the difference was statistically significant（χ2= 22.22, P 〈0.05）; the Barthel index score good rate of total hip group（100.0%, 60/60） was better than that of half hip group（73.3%,44/60）, the difference was statistically significant（χ2= 30.81, P 〈0.05）; the postoperative occurrence rates of femoral head ischemic necrosis, femoral neck to shorte, infection in total hip group were lower than those in half hip group（0.0% vs 10.0%, 0.0% vs 6.7%, 0.0% vs13.3%）, the differences were statistically significant（χ2= 10.53, 6.93, 14.25, P 〈0.05）. Conclusion The clinical effect of treatment agedness femur neck fracture by total hip replacement surgery is obvious, prognosis is good,which is worthy to be used in clinic.
