摘要
目的探索临床药师个体化用药服务模式。方法基于现有的药学服务模式,以个体化用药指导为方向,临床药师通过对病人用药教育、协助医师完成个体化治疗方案、提供技术支持以及完成相关培训工作,实施个体化用药服务。结果开展个体化用药服务,能够提高病人用药的依从性、保证临床合理用药,真正体现"以病人为中心"的服务理念。结论开展临床药学个体化用药服务,可以为病人提供更优、更适宜的医疗保健服务。
Objective To explore a service pattern of individualized medication of clinical pharmacists. Methods Base on the existing pharmaceutical care mode and the gnidance of individualized medication, clinical pharmacists helped physicians corn plete individualized treatment through education of medication in patients, provided technical support to finish the related training and implement individualized medication. Results Carrying out individual treatment service improved the medication compliance of the patients, ensured the rational clinical drug use and really reflected the service concept of taking the patients as the centre. Conclusion Clinical pharmacy for personalized medication service can provide the patients with better and more suitable medical and health care service.
