射频消融联合人工骨椎体成形术治疗脊柱转移瘤28例 被引量:4

Radiofrequency ablation combined with artificial bone vertebral plasty in treatment of spinal metastatic tumors in 28 cases

摘要 目的探讨射频消融(RFA)联合注射用人工骨椎体成形术(PVP)治疗脊柱转移瘤临床疗效。方法对28例脊柱转移瘤患者进行多点RFA治疗后,注射自固化磷酸钙人工骨行PVP。观察手术并发症、术后椎体稳定性及复发情况。分别采用视觉模拟评分(VAS)和脊柱ODI评分,对比术前及术后1、3、6、9、12个月患者疼痛及脊柱功能。结果所有患者均手术成功,无严重并发症。术后随访12个月内,所有病例均无椎体肿瘤复发征象,无骨折发生。术后12个月内,Cobb角与术前相比无显著改变。VAS和ODI评分在术后1个月均较术前显著降低(P<0.01),并维持稳定水平;术后12个月与术后1个月相比无显著差异(P>0.05)。结论 RFA联合人工骨PVP治疗脊柱转移瘤效果显著,可长期维持椎体稳定性。 Objective To explore the clinical effect of radiofrequency ablation (RFA) combined with percutaneous vertebral plasty(PVP) in the treatment of spinal metastatic tumors. Methods After multi-point RFA treatment was provided for the 28 patients with spinal metastatic tumors, self-setting calcium phosphate cement (CPC)was injected for PVP; and then the postoperative complications and postoperative vertebral stability and recurrence were observed. By use of visual analogue score (VAS) and spinal ODI score, pain and spinal function of the patients before the operation and within one month, three months, six months, nine months, and twelve months after the operation were compared and analyzed. Results All patients were operated successfully, without serious complications. During the follow-up period of twelve months, there were no spinal metastasis recurrence and no frature in all cases. Within twelve months after the operation, Cobb angles had no significant change from the preoperative ones. Within one month after the operation,the VAS and ODI scores decreased significantly (P 〈 O. O1 )and maintained a stable level, and there were no significant difference between the cases within twelve months and one month after the operation ( P 〉 0.05 ). Conclusion RFA combined with artificial bone PVP in treatment of spinal metastatic tumors has obvious effect, and can maintain long-term vertebral stability.
