肩关节镜下带线锚钉缝合修复上盂唇与单纯清创治疗Ⅱ型SLAP损伤疗效比较 被引量:3

摘要 目的比较肩关节镜下带线锚钉缝合修复上盂唇与单纯清创治疗Ⅱ型上盂唇前后(SLAP)损伤的疗效差异。方法回顾性分析2010年6月至2013年4月大连大学附属中山医院骨四科行肩关节镜下SLAP损伤治疗24例患者的临床资料,将其中行关节镜单纯肩关节上盂唇清创术者分为对照组(10例),行关节镜下带线锚钉缝合修复上盂唇术者分为观察组(14例)。两组患者术后进行相同康复训练。24例患者均获得随访至1年。采用美国肩肘外科医师评分(ASES)及加州大学洛杉矶分校(UCLA)评分进行功能评估。结果术前两组患者ASES、UCLA评分[观察组分别为[(46.90±9.28)、(18.02±1.98)分,对照组分别为(47.86±9.80)、(18.56±2.12)分]比较,差异均无统计学意义(P>0.05),术后1年两组患者ASES、UCLA评分均较术前显著改善[观察组分别为(92.08±5.80)、(33.42±1.52)分,对照组分别为(87.52±5.84)、(31.15±1.82)分],差异均有统计学意义(P<0.05),且观察组ASES、UCLA评分均高于对照组,差异均有统计学意义(P<0.05);两组患者均未发生血管、神经损伤,感染等并发症。结论肩关节镜下带线锚钉缝合修复上盂唇较单纯清创治疗Ⅱ型SLAP损伤具有更好的临床疗效。 Objective To approach effect comparison between anchor lip suture and repair with a line under arthroscop-ic shoulder and simple debridement suture repair in treatment with type Ⅱ SLAP lesion. Methods A retrospective analysis was conducted in 24 patients underwent SLAP lesions under arthroscopic in the department of department the fourth orthopedics,Affiliated Zhongshan Hospital of Dalian University from June 2010 to April 2013. They were divided into the control group,10 cases with simple anchor lip debridement in the shoulder and the observation group,14 cases with anchor lip suture and repair with a line under arthroscopic shoulder. The two groups attended the same rehabilitation training after the operation. All the objects were followed up for one year. It was assessed with American Shoulder and Elbow Surgeons(ASES) and University of California Los Angeles(UCLA). Results There was no statistical significance in ASES and UCLA scores of the two groups(P〉0.05)[(46.90±9.28),(18.02±1.98)in the observation group vs.(47.86±9.80),(18.56±2.12) in the control group]. The ASES and UCLA scores before the operation were significantly improved after the operation between the two groups,which had statistical significance in difference(P〈0.05),and the UCLA score in the observation group was higher than that of the control group,whose difference was statistically significant(P〈0.05);The ASES score in the observation was higher than that of the control group,whose difference was statistically significant(P〈0.05). The complications such as blood vessel,nerve injury and infection didn′t occurr. Conclusion The treatment of anchor lip suture and repair with a line under arthroscopic shoulder had a better effect in clinic.
