摘要
目的探讨多排螺旋CT(MDCT)图像观察对食管胃结合部腺癌(AEG)手术方式选择的应用价值。方法通过MDCT图像观察对入组的62例AEG患者进行Siewert分型,并分析其与手术方式的关系。结果62例患者中,SiewercⅠ型1例(1.6%)、Ⅱ型49例(79.0%)、Ⅲ型12例(19.4%)。手术路径:1例SiewertⅠ型AEG患者采用经胸路径;49例SiewertⅡ型AEG患者中,采用经左胸切口入路16例(32.7%)、经上腹正中切口入路30例(61.2%),经胸腹联合切口入路3例(6.1%);12例SiewertⅢ型AEG患者均采用经腹路径。结论术前MDCT图像观察有利于AEG手术方式的正确合理选择。
Objective To investigate the application value of multidetector CT（ MDCT）in the operation method choice of adenocarcinoma of the esophagogastric junction（ AEG）. Methods Sixty-two cases of AEG were typed by using Siewert typing method,and the relationship with surgery path were analyzed too. Results Of the 62 cases of AEG,Siewert type Ⅰ1 case（1. 6%）,Siewert type Ⅱ49 cases（79. 0%）,Siewert type Ⅲ12 cases（19. 4%）, Surgery path：1 case with Siewert typeⅠwas treated with the thoracic path;of the 49 cases with Siewert typeⅡ,16 cases（32. 7%）were treated with the chest path,30 cases（61. 2%）with the abdominal path,3 cases（6. 1%） with the chest-abdominal path;12 cases with Siewert type Ⅲ were treated with abdominal path. Conclusion MDCT make for the operation method choice of AEG.
