摘要
目的探讨补肾活血方剂对老年骨质疏松引发的椎体压缩性骨折患者术后骨密度与机体功能的疗效与作用,优化临床治疗方案。方法将2011年6月至2013年12月收治的113例骨质疏松性椎体压缩性骨折患者随机单盲分为观察组59例和对照组54例,所有患者均行椎体成形术,术后对照组口服阿法骨化醇减缓骨质疏松,观察组在对照组基础上加服自拟补肾活血方剂,每日1剂,疗程均为3个月。对比两组治疗前、术后3和6个月的疼痛程度、骨密度与机体功能。结果术后3个月,观察组疼痛视觉模拟(VAS)评分、脊柱后凸角度、Dswestry功能障碍指数(ODI)、骨密度值略优于对照组,但差异无统计学意义(P>0.05);术后6个月,两组VAS评分、脊柱后凸角度、ODI指数、骨密度值分别为(2.6±1.0)比(3.1±1.0),(27.1±5.6)比(24.9±5.3),(21.2±8.9)%比(27.4±8.4)%,(-3.0±-0.9)比(-3.4±-1.1),差异均具有统计学意义(P<0.05)。治疗期间,两组转氨酶升高、血钙升高、便秘发生率分别为16.9%比13.0%,6.8%比9.3%,20.3%比11.1%,差异均无统计学意义(P>0.05)。结论补肾活血方能在较大程度上减轻老年压缩性椎体骨折患者术后疼痛症状,改善骨代谢,增强骨密度,减轻骨折引发的机体功能障碍。
Objective To observe and explore the effect of Bushen Huoxue Decoction on the bone mineral density and body function after vertebroplasty in the elderly patients with compressive fracture,and to optimize the clinical treatment scheme. Methods 113 pa- tients with osteoporotic vertebral compressive fracture admitted to our hospitals from June 2011 to December 2013 were divided into the observation group（59 cases） and the control group（54 cases） by the random single-blind method. All the patients underwent vertebro- plasty. The control group was postoperatively given oral alfacalcidol for alleviating osteoporosis, while on the basis of the control group treatment the observation group was added with self-designed Bushcn Huoxue Decoction, 1 agent daily. The treatment course was 3 months. The pain degree,bone mineral density and body function before operation and at postoperative 3,6 months were compared be- tween 2 groups. Results The VAS scorc,kyphosis angle,ODI index,bone mineral density values at postoperative 3 months in observa- tion group were slightly superior to those in the control group,but the difference had no statistical significance （P 〉 0. 05 ） ; which at postoperative 6 months in the two groups were （2.6±1） vs （3.1±1）,（27. 1±5.6） vs （24.9±5.3）,（21.2±8.9）% vs （27.4±8.4）%, （ - 3.0±0. 9） vs （ - 3.4±- 1.1）, the differences were all statistically significant（ P 〈 0. 05）. During treatment period, the propor- tions of ALT increase,serum calcium increase and constipation in the observation group and the control group were 16. 9% vs 13.0% , 6.8% vs 9.3% and 20. 3% vs 11. life, the differences between the two groups were not statistically significant（ P 〉 0. 05）. Conclusion Bushen Huoxue Decoction can alleviate the postoperative pain symptom in a large extent for the elderly patients with com- pressive fracture ,improve the bone metabolism,enhance the bone mineral density and reduce the body dysfunction caused by fracture.
