摘要
目的探讨脑桥梗塞患者脑桥梗塞体积与患病3个月后预后情况的关联性及脑桥梗塞的临床特点。方法选取2009年1月至2013年1月30例脑桥梗塞患者,按照头颅核磁及发病时间显示结果,分为超急性期(3-6h)6N,急性期(7-72h)14例,亚急性期(94—336h)10例。30例急性脑桥梗塞均行头颅MRI检查,并对其病灶位置分布、神经系统体征、神经功能缺损程度等指标进行分析。同时在其头颅核磁弥散加权成像(DWI)上测量脑桥梗塞体积量值,结合中国脑卒中临床神经功能缺损程度评分量表(CSS)系统评估患者入院第1天情况和3个月的情况。3个月后对患者神经功能缺损程度行改良Rankin评分(MRS)评定。结果患病各期患者的脑桥梗塞体积与入院时所做的神经功能缺损评分呈正相关(r=0.89,0.95,0.92,P〈0.01),脑桥梗塞体积与患者预后3个月评分呈正相关(r=0.87,0.89,0.91,P〈0.01)。结论脑桥梗塞体积的大小与入院1d内所做的神经功能缺损评分密切相关,也与病后3个月预后情况密切相关,可作为其预后的单变量预测因素。
Objective To discuss the clinical characteristics ofpontile poplexy and the correlation analysis of the volume ofpontile poplexy and prognosis. Methods From Jan. 2009 to Jan. 2013, we selected 30 cases of patients with pontile poplexy from neurology ofbayannaoer city hospital to do the study. According to the results of head MRI and onset, the study patients were divided into super acute period (onset 3-6h, n=6), acute period (onset 7-72h, n=14) and sub-acute period (onset 94-336h, n=10), respectively. All patients were examined by head MRI, and then we analyzed the distribution of nidus, the symptoms of nervous system, neurological function deficits scale (through Chinese Stroke Scale score). Meanwhile, measuring the volume of pontile poplexy on the basis of Head MRI diffusion weighted imaging; we evaluated the patient' s condition after entering the hospital the first day and three months by CSS, and neurological function deficits scale through modified Rankin scale (MRS) after 3 months. Results There was a positive correlation between the volume of pontile poplexy in all patients with different DWI and neurological function deficits scale (r=0.89, 0.95, 0.92, P〈0.01), and MRS scale (r=0.87, 0.89, 0.91, P〈0.01). Conclusions There were close correlationships between the volume of pontile poplexy and neurological severity score both on addimssion and the prognosis after 3 months, and could be used as a single variable to predict the prognosis factors.
