摘要
目的初步探讨寻常痤疮的发病与中医体质的相关性。方法 318例寻常痤疮患者在两名中医专业调查人员的协助下,完成《中医体质分类与判定表》和《寻常痤疮患者一般情况登记表》的填写,记录患者一般资料和体质类型,分析不同年龄段、性别、痤疮级别、精神因素、遗传情况下患者中医体质类型的分布情况,并进行统计学分析。结果 318例患者中,湿热质所占的比例最大,有258例次占23.54%,其后分别为阴虚质>痰湿质>阳虚质>气郁质>气虚质>血瘀质>特禀质>平和质(P<0.05);阳虚质、血瘀质、气郁质这3种体质在女性寻常痤疮患者比例高于男性患者(P<0.05);阳虚质在青春期后患者比例高于青春期患者(P<0.05);各种体质类型在不同级别寻常痤疮中的构成比差异均无统计学意义(P>0.05);气虚质、气郁质在受精神因素影响患者的百分比高于不受精神因素影响的患者(P<0.05);各种体质类型在有遗传因素和无遗传因素患者中的构成比差异无统计学意义(P>0.05)。结论湿热质是寻常痤疮发病的主要体质类型,部分体质类型的分布受性别、年龄及精神因素等方面的影响,而遗传因素、痤疮的严重程度的影响不大。
Objective To investigate the correlation of the onset of acne vulgaris and Traditional Chinese Medicine（ TCM） constitution types. Methods With the assistance of two TCM professional investigators,318 patients with acne vulgaris completed the ‘TCM Constitution Type Classification and Decision Table＇and ‘General Information Registration Form for Acne Vulgaris Patients＇ to recorde the general information and constitution types of the patients. The distribution of TCM constitutions was analyzed with the factors of age,gender,acne levels,mental factors and genetic condition with statistical analysis. Results In 318 patients,dampness-heat type took the largest proportion,with 258 cases（ 23. 54%）,followed by yin-deficiency type,phlegm-dampness type,yang-deficiency type,qi-stagnation type,qi-deficiency type,blood-stasis type,special type and gentle type（ P〈0. 05）. Yangdeficiency type,blood-stasis type and qi-stagnation type had larger proportions in female than in male patients（ P〈0. 05）. Yang-deficiency type took a higher percentage in patients after puberty than during puberty（ P〈0. 05）. The difference of constituent ratio of each constitution type in different levels of acne vulgaris and with or without genetic factors showed no statistical significance（ P〈0. 05）. The percentage of qi-deficiency type and qi-stagnation type in the patients with mental factors were larger than those without（ P〈0. 05）. Conclusion Dampness-heat type is the main constitution type of acne vulgaris. The distribution of some types were influenced by gender,age and mental factors and little by genetic factors and severity of the acne.
