多b值磁共振扩散加权成像在颈部淋巴结病变中的应用价值 被引量:2

摘要 目的:探讨磁共振多b值扩散加权成像技术在颈部淋巴结良恶性病变中的鉴别诊断价值。方法:回顾性分析55例单侧或双侧颈部淋巴结肿大患者的常规磁共振成像及多b值扩散加权成像,b值分别取0、400、600、800,、1000、1500、2000 s/mm2。病变分为良性病变组共23例,恶性病变组32例,所有病例均经手术或穿刺活检证实。结果:55例患者术前行常规MRI和DWI检查。当b值≤1000 s/mm2,DWI的SNR在一定范围内随b值的增加而升高,当b=1000 s/mm2达到最高峰;当b值>1000 s/mm2时,随b值的增加,SNR逐渐下降。当b=1000 s/mm2时,良恶性病变平均ADC值差异有统计学意义(P<0.05)。ROC曲线分析显示,ADC阈值为1.07×10-3 mm2/s时,对区分良恶性病变的灵敏度、特异度和曲线下面积分别为85.6%、82.3%和0.896。结论:b值取1000 s/mm2时,能取得良好的图像质量,并且对于鉴别颈部良恶性淋巴结病变有较高的诊断效能。 Objective:To investigate the applying value of multiple b-values diffusion weighted imaging on differentiation of benign and malignant cervical lymph node lesions.Method:55 patients with 23 benign cervical lymph node lesions and 32 malignant ones underwent diffusion-weighted imaging at 3.0 T MRI with different b values of 0,400,600,800,1000,1500 and 2000 s/mm2.All lesions were confirmed by pathology.Result:When b-value≤1000 s/mm2,the SNR of DWI increased as the b-value increased,and when b-value =1000 s/mm2,the SNR reached its climax,when b-value &gt;1000 s/mm2,the SNR of DWI decreased as the b-value increased.With b value of 1000 s/mm2,average ADC value of benign lesion was higher than malignant lesion,among them the differences were statistically significant(P〈0.05).Average ADC had an acceptable value on differentiation of benign and malignant cervical lymph node lesions with optimal cutoff point of 1.07×10-3 mm2/s,yielding a sensitivity,specificity and the area under the AUC were 85.6%,82.3% and 0.896,respectively.Conclusion:For good image quality and valid differentiation between malignant and benign lymph nodes,the optimized b value of DWI at 3.0 T for cervical lymph nodes is 1000 s/mm2.
