摘要
目的探讨照顾者反应对维持性血液透析(maintenance hemodialysis,MHD)患者自我感受负担(self perceived burden,SPB)的影响。方法采用SPB量表和中文版照顾者反应评估量表对365例MHD患者及其照顾者进行问卷调查,采用Pearson相关分析法和分层回归分析法探讨照顾者反应对MHD患者SPB的影响。结果 1MHD患者SPB得分为(35.89±5.25)分,328例(89.86%)存在SPB;2Pearson相关分析显示,MHD患者SPB评分与照顾者反应中的健康问题、时间安排受打扰、经济问题、家庭支持缺乏均呈正相关;与自尊呈负相关(P<0.05)。2多元回归分析显示,家庭收入、需要照护时间、主要照顾者与患者关系和照顾者反应均为MHD患者SPB的影响因素。结论照顾者反应是MHD患者SPB的影响因素,医护人员可以对照顾者反应进行综合干预,以降低MHD患者的SPB水平。
Objective To investigate the impact of caregiver reaction on the self-perceived burden（SPB） in mainte- nance hemodialysis （MHD） patients. Methods Totally 365 MHD patients and their caregivers were investigated using the Self-Perceived Burden Scale and the Chinese version Caregiver Reaction Assessment Scale. Pearson correla- tion analysis and hierarchical regression analysis were used to explore the impact of caregiver reaction on the SPB in MHD patients. Results The average score of SPB was（35.89±5.25）,and 328 patients（89.86%） suffered SPB. Pearson correlation analysis showed that the score of SPB was positively correlated with health problems,time arrangements disturbed,economic problems and lack of family support,while negatively correlated with self-esteem. Hierarchical regression analysis showed that family income,caring time,the relationship between primary caregiver and patients, and caregiver reaction were the influence factors of SPB in MHD patients. Conclusion The caregiver reaction is an important influencing factor of SPB in MHD patients.
