摘要 目的 系统评价5-氨基酮戊酸光动力疗法(ALA-PDT)治疗尖锐湿疣的临床疗效及安全性.方法 计算机检索Cochorane Library、PubMed、EMBASE、CNKI、CBM、VIP等数据库,检索的日期从1990年至2012年5月.收集所有5-氨基酮戊酸光动力疗法治疗尖锐湿疣的临床随机对照试验(RCT),并对其进行系统评价,治疗组为ALA-PDT组,对照组为CO2激光组.结果 最终3个随机对照试验完全符合本系统评价的纳入标准,纳入患者是597例,Meta分析显示ALA-PDT治疗尖锐湿疣治愈率高,为90.0%,对照组为71.0%,其差异有统计学意义(RR=1.27,95%CI为1.15~1.42,P<0.000 01).局部不良反应,治疗组8.4%,对照组66.5%,其差异有统计学意义(RR=0.13,95%CI为0.10~0.18,P<0.000 01).结论 ALA-PDT治疗尖锐湿疣治愈率高,局部不良反应轻微,患者依从性较好,是一种安全有效的治疗方法. Objective To assess the efficacy and safety of topical 5-aminolevulinic acid photodynamic therapy (ALA-PDT) in curing genital warts. Methods Cochorane Library (1990-2012), PubMed (1990-2012), EMBASE (1990-2012), Chinese Biomedical Literature Database (CBM) (1990-2012), China National Knowledge Infrastructure (CNKI) (1990-2012), VIP database (1990-2012), as well as reference lists of relevant studies were searched. Two reviewers independently screened searched studies, extracted data and evaluated their ethodological qualities. RevMan 5.1.0 software was used for meta-analysis. Results Three RCTs involving 597 people were included. Among them, three studies demonstrated the complete response rate of locally-used photodynamic therapy as compared to CO2 laser for treating genital warts. Based on meta-analysis, the rate of complete response of the two interventions differed significantly (ALA-PDT: 90.0%; CO2 laser. 71.0%). Three had statistical significant difference between the two groups (RR=l.27, 95%CI:1.15-1.42, P 〈0.000 01). Local reactions, treatment group was 8.4% ,66.5%in the control group. There was statistically significant difference (RR =0.13, 95% CI:0.10-0.18, P〈0.000 01). Conclusion ALA-PDT tends to be a fairly well-tolerated form of therapy. According to different routes of administration, ALA- PDT appears to be much more effective than CO2 laser for treating genital warts in either improving the complete response rate or reducing the side-effect rate for the treatment of genital warts.
