摘要
目的:探讨应用负压封闭引流技术联合皮瓣转移修复足踝部皮肤缺损的方法及疗效。方法:2003年10月~2013年10月,对20例足踝部皮肤缺损采用负压封闭引流技术联合皮瓣转移修复,其中男14例,女6例。年龄5~62岁,平均36岁。损伤原因:烧伤10例,机器伤5例,车祸伤3例,感染2例。其中行负压封闭引流技术1次12例,2次6例,3次2例,皮瓣选择:外踝上皮瓣8例8个皮瓣,切取皮瓣范围5cm×4cm^13cm×8cm;逆行腓肠神经营养皮瓣12例12个皮瓣,切取皮瓣范围6cm×4 cm^18cm×9cm;修复部位包括足背、足跟和内、外踝,皮肤缺损范围6cm×4cm^15cm×8cm。结果:术后外踝上皮瓣完全成活6例,完全坏死1例,尖端部分坏死1例。逆行腓肠神经营养皮瓣完全成活10例,其余2例尖端出现表皮坏死,经换药后痊愈。术后经1月~5年随访,皮瓣皮肤质地、厚度与足踝部相接近。结论:负压封闭引流技术联合皮瓣修复足踝部皮肤缺损是一种较好治疗方法,具有操作简单,创面感染率低,成功率高,不损伤主要血管,适合在基层医院开展。
Objective To explore the application of closed negative pressure drainage technology transfer to repair the skin defect of ankle joint flap method and curative effect. Methods In October 2003 to October 2013,20 cases of foot skin defect with the closed negative pressure drainage technology joint flap transfer,including 14 cases of male, female 6 cases.Age 5-62, an average of 36 years old. Damage reason: 10 cases of burn,machine injury in 5 cases, car accident injury 3 cases, infection in 2 cases.Line of closed negative pressure drainage technology one 12 cases,6 cases,2 times 3 times in 2 cases,the flap option: external ankle flap 8 cases of 8 flap, cut the flap harvest range 5cm×4cm-13cm×8cm.Retrograde sural nerve nutritional flap 12 cases 12 flap, cut the flap harvest range 6cm×4cm-18 cm×9cm.Repair parts,including interior and external ankle foot dorsum, heel,and skin defect range 6cm×4cm to 15cm×8cm. Results postoperative external ankle completely transplanted tisssues survived 6 cases,complete necrosis in 1 case,tip necrosis in 1 case.10 cases of retrograde sural nerve nutrition flaps survived completely and the rest 2 cases of cutting-edge epidermal necrosis,after switching to heal. After 1 month to 5 years follow-up,flap with foot close to the skin texture,thickness. Conclusion closed negative pressure drainage technology joint flap to repair the skin defect in the ankle is a better treatment method, with simple operation,wound infection rate is low,high success rate, does not damage the main blood vessels,suitable for primary hospitals.
