摘要
目的探讨血清神经元特异性烯醇化酶(NSE)浓度与重症手足口病患儿神经系统损害的相关性,为提高该类患儿的诊疗质量提供指导依据。方法将确诊的125例重症手足口病患儿,按照血清NSE浓度水平,分为A组(〈16.3ng/m L)25例,B组(16.3-32.6ng/m L)70例,C组(〉32.6ng/m L)30例,比较三组患儿的临床指标与表现。结果三组患儿在脑脊液细胞阴转率、脑电图异常率及磁共振异常率等临床指标比较,差异均有统计学意义(P〈0.05),其中,A组患儿的脑电图异常率、磁共振异常率最低,脑脊液细胞阴转率最高,而C组患儿的脑电图异常率、磁共振异常率最高,脑脊液细胞阴转率最低;随着血清NSE浓度的升高,脑脊液蛋白、氯离子(Cl-)、糖等增高率与脑膜刺激征、抽搐等发生率均逐渐上升,A组最低,C组最高,三组患儿以上指标对比,差异均有统计学意义(P〈0.05)。结论血清NSE浓度与重症手足口病患儿神经系统损害程度存在密切相关性,对重症手足口病患儿的血清NSE浓度进行检测,可反映神经系统损害的严重程度。
Objective To discuss the correlation of serum level of neuron specific enolase (NSE) concentration with nervous system damage in children with severe hand-foot-mouth disease(HFMD). Methods One hundred and twenty-five HFMD children were, according to the concentration of serum NSE level, divided into Group A comprised 25 cases with serum NSE level of 〈16.3ng/mL, Group B comprised 70 cases of with serum NSE level of 16.3 - 32.6ng/mL and Group comprised 30 cases with serum NSE level of 〉32.6ng/mL, clinical parameters of the children in three groups were compared. Flesults The negative conversion rates of cerebrospinal fluid cell count, the abnormal rates of EEG and MRI abnormality and other clinical indicators of children in three groups were significantly different (P 〈 0.05), the abnormal rate of EEG, the rate of abnormal MRI of children in group A was the lowest, the negative conversion rate of eerehrospinal fluid cell count was the highest, while the abnormal rate of EEG, the rate of abnormal MRI of children in Group C were the highest, the negative conversion rate of cerebrospinal fluid cell count was the lowest; With the increasing of serum NSE concentration the parameters of cerebrospinal fluid, protein, chloride ion (C1-), sugar,etc, increasing rate and meningeal irritation sign, The incidence of convulsions were gradually increased ,being the lowest in Group A and highest in group C and significant differences were noticed among three groups (P 〈0.05). Gonclusiorl There is a close correlation between serum NSE concentration and severe HFMD children with nervous system lesion and the detection of serum NSE concentration in HFMD children can disclose the severity of nervous system lesion.
