摘要
目的:探讨超声引导下肺穿刺活组织检查(US-PCNB)对良恶性肺周围性病变的鉴别诊断价值。方法:45例肺周围性占位病变患者行常规超声检查,了解病变的大小、回声、毗邻以及血供情况,结合CT,MRI等其他检查结果综合分析,在超声引导下用一次性自动活组织检查针穿刺进行活组织检查,所取标本送病理学检查。结果:45例患者,3例因取材少或未取到病变处而未能明确诊断,其余病例与病理结果对照,良性病变16例,其中结核6例,炎性8例,炎性假瘤1例,肺脓肿1例;恶性病变26例,其中腺癌7例,鳞癌15例,小细胞肺癌4例。组织学确诊阳性率为93%(42/45),且未出现血胸等重大并发症。结论:超声引导下肺周围性病变穿刺活组织检查安全、简便、快速、创伤小、无放射性、诊断率高、并发症少,对鉴别肺周围性良恶性病变具有良好的临床应用价值。
Objective：To investigate the value of ultrasound guided lung puncture biopsy（US - pCNB）in the diagnosis of benign and malignant lung pathology. Methods：Forty - five patients with lung around pathology undergoing routine ultrasound examination,to know the size of the lesion,echo,adjacent and blood supply situation,comprehensive analysis combined with other tests such as CT,MRI,ultrasound guided with one - time automatic biopsy needle biopsy,the specimens by pathological examination. Results：Forty - five patients,3 cases without get specimens failed to diagnosis,the remaining contrast with patho-logical results,benign lesions 16 cases,6 cases tuberculosis,8 cases inflammation,1 case inflammatory pseudotumor,1 case pulmonary abscess;Malignant lesions 26 case,7 cases adenocarcinoma,15 cases squamous carcinoma,4 cases small cell lung cancer. histologic diagnosis positive rate was 93%（42 / 45）,and does not appear hemothorax and other major complications. Conclusion：Ultrasound guided percutaneous lung peripheral lesions biopsy,safe,convenient,fast,small trauma,no radioactivi-ty,high diagnostic rate,fewer complications,has good clinical application value.
