摘要
目的 :观察中医辨证施治妇科盆腔肿瘤放射性损伤的治疗效果。方法:将178例门诊就诊妇科肿瘤放疗患者随机分为中医治疗组138例和对照组40例,均行单纯盆腔根治性放疗,治疗组经辨证后分为湿热蕴结证、脾虚痰湿证、肝郁脾虚证、痰瘀互结证、肝肾不足证5型,分别予连朴饮、二陈汤合平胃散、柴胡疏肝散、血府逐瘀汤合二陈汤、一贯煎合二至丸加减;对照组仅在出现急性放射性肠炎或放射性膀胱炎时给予一般对症处理,口服思密达、氟哌酸或谷参肠安,比较两组患者治疗前后放射性损伤分级和中医证候积分及疗效。结果:治疗组发生2级放射性损伤的比例为10.9%(15/138),明显少于对照组40.0%(16/40),两组差异有统计学意义(P<0.05);中药治疗组证候积分及证候疗效与对照组比较均有统计学意义(P<0.01,P<0.05),中药治疗组各证型患者经治疗后,与治疗前比较证候积分值差异有统计学意义(P<0.05)。结论:中医辨证施治妇科盆腔肿瘤放射性损伤有确切疗效,能明显减轻放疗毒副反应,控制并发症,提高患者生活质量。
Objective： To observe the effect of treatment based on syndrome differentiation on radioactive lesion due to radiotherapy for gynecological pelvic tumor. Methods： The 178 cases of outpatients with gynecological tumor and radiotherapy were randomly divided into TCM treatment group with 138 cases and control group with 40 cases,two groups were treated by pure radical radiotherapy for pelvic. After syndrome differentiation,the treatment group was divided into damp-heat stagnating syndrome,spleen deficiency and phlegm damp syndrome,liver stagnation and spleen deficiency syndrome,phlegm stagnation with blood stasis syndrome,liver and kidney impairment syndrome. The 5 syndrome types were given respectively lianpu drink,erchen decoction plus pingwei power,chaihu shugan powder,Xuefuzhuyu decoction plus erchen decoction,yiguan decoction plus erzhi pill addition and subtraction. While the control group was given general symptomatic treatment i. e. oral smecta,norfloxacin or compound glutamin entersoluble capsules,only when the acute radioactive enteritis or radioactive cystitis occurred. Radioactive lesion gradations,TCM syndrome scores and effects before and after treatment were compared in patients of two groups. Results： The proportion of Ⅱclass radioactive lesion occurred in the treatment group was 10. 9%（ 15 /138）,significantly less than 40. 0%（ 16 /40） of control group. The difference between two groups was statistically significant（ P〈0.05）. Compared with control group,syndrome scores and syndrome effects of TCM treatment group were statistically significant（ P〈0. 01,P〈0.05）. Compared the treated patients with the various syndromes in TCM treatment group with before treatment,the difference of symptom scores values was statistically significant（ P〈0.05）. Conclusion： TCM treatment based on syndrome differentiation has exact effect on radioactive lesion due to radiotherapy for gynecological pelvic tumor,it can obviously relieve the toxic side effects due to radiotherapy,control th
