摘要
目的研究颈椎病慢性疼痛患者经中医手法治疗后,与临床症状相关的静息态脑功能默认网络的变化。方法研究对象为10例颈椎病慢性疼痛的患者,经12周中医推拿治疗,治疗前后予以颈椎病量表评估,并采用西门子3.0T磁共振仪采集静息态脑功能磁共振数据,应用独立成分分析提取出默认网络(Default Mode Network,DMN),采用支持向量机(Support Vector Machine,SVM)算法,找出对区分治疗前后的DMN最有意义的脑区,对这些区域与临床量表的相关性进行分析。结果与病情呈正相关的脑区有左侧额下回三角区,左额中回眶部,左舌回和右侧楔前叶;与病情呈负相关的脑区包括左侧直回,双侧中央后回,右侧额上回,右顶下回。结论中医推拿手法治疗可能通过调整患者的DMN,增强DMN与感觉和执行相关皮层的连接,抑制其与认知、视觉及记忆皮层的联系而改善临床症状。
Objective To explore the clinical symptom associated changes of default Mode Network of patients suffering from chronic cervical radicular pain (CCRP) after traditional Chinese massage therapy.Methods 10 patients of CCRP received 12 weeks' Chinese massage therapy.Scale of cervical spondylosis was evaluated and resting state functional magnetic resonance scan was given in a 3.0T SIEMENS scanner before and after the treatment.Default Mode Network (DMN) was extracted using independent component analysis.Support Vector Machine(SVM) algorithm was applied to find the most significant brain regions to distinguish the pre therapy and post therapy data of DMN.Correlation analysis was then adopted between DMN in these regions and clinical scale.Results The brain regions positively related to disease included left triangular part of inferior frontal gyrus,left orbital part of middle frontal gyrus,left lingual gyrus,and right precuneus.While negatively related brain regions covered right middle and superior frontal gyrus,right inferior parietal gyrus,and right post central gyrus.Conclusion Traditional Chinese massage could reduce the CCRP patients' clinical symptoms by enhancing DMN connection to somatosensory and executive cortex,meanwhile inhibiting DMN connection to cognitive,visual,and memory cortex.
