摘要
目的应用超声心动图分析孕期体质量变化对正常孕期妇女心脏结构及功能影响。方法选择医院产科门诊正常初孕妇女120例,根据怀孕期间体质量变化程度分为增重不足组、增重适宜组及增重过多组各40例,3组患者进行心脏超声心动图检查。比较不同体质量变化组孕期妇女的心脏超声结构及功能参数:左心室舒张末期内径(LVEDd)、左心室收缩末期内径(LVEDs)、左心室射血分数(LVEF)、左心房内径(LAD)、二尖瓣舒张早期充盈速度/二尖瓣舒张晚期充盈速度(E/A)、二尖瓣舒张早期充盈速度/心肌舒张早期速度峰值(E/Ea),分析体质量的变化对孕期妇女心脏结构及功能影响。结果增重过多组孕妇超声参数LVEDd、LVEDs、LAD、E/Ea均大于增重适宜组,LVEF、E/A值小于增重适宜组,差异均有统计学意义(P<0.05);增重不足组与增重适宜组比较,LVEDd、LVEDs、LVEF、LAD、E/Ea差异无统计学意义(P>0.05),但增重不足组E/A值大于增重适宜组,差异有统计学意义(P<0.05)。结论正常妊娠妇女心脏结构及功能负荷随着体质量的变化而不同,孕期体质量变化过大,增加心脏负荷及影响心脏正常结构,其中心脏舒张功能影响较明显。
Objective Applying the ultrasonic Heartbeat diagram to analyse the effects of pregnancy body mass on cardiac structure and function. Methods 120 early pregnant women were selected. According to the degree of change in body mass during pregnancy,the 120 pregnant women were divided into weight gain problem group,weight gain suitable group and excessive weight gain group,each of 40 cases. All the pregnant women took cardiac ultrasound beckoning graph examination, and to compare the left ventricular end diastolic diameter( LVEDd),left ventricular end systolic diameter( LVEDs),left ven-tricular ejection fraction( LVEF),left atrial diameter( LAD);mitral early diastolic velocity of mitral/late diastolic filling veloci-ty( E/A),mitral early diastolic filling velocity/myocardial peak early diastolic velocity( E/Ea). Then analysed the influence of body weight on cardiac structure and function in pregnant women. Results The ultrasound parameters LVEDd,LVEDs, LVEF,LAD,E/Ea of excessive weight gain group were greater than the weight of suitable group,and E/A value is less than the weight of suitable group,the difference were statistically significant(P〈0. 05);The differences of LVEDd,LVEDs,LVEF, LAD,E/Ea between weight gain problem group and weight gain suitable comparison group were not statistically significant( P〉0. 05),but E/A value of the weight problem group was greater than that of the weight of suitable group,the difference was sta-tistically significant(P〈0. 05). Conclusion The body mass can affect the normal pregnancy women cardiac structure and function of load. If the pregnancy body mass change is too large,the heart load will be increased and cardiac structure will be influenced,and the cardiac diastolic function is affected obviously.
