摘要
目的:观察益生菌在幽门螺杆菌(Helicobacter pylori,H.pylori)感染消化性溃疡(peptic ulcer,PU)中的治疗效果.方法:从广西壮族自治区江滨医院抽取2013-05/2014-05消化内科收治的H.pylori感染P U患者132例,按照数字随机法将患者分成两组,即观察组和对照组,每组分别66例,给予观察组患者益生菌联合三联疗法进行治疗,给予对照组患者传统的三联疗法进行治疗,通过1m o的治疗,对所有患者行14C尿素呼气试验检查,查看H.pylori根治情况,并进行胃镜复查,查看溃疡愈合情况,对比两组患者治疗效果及消化道症状改善情况.结果:两组患者经过治疗后,经过14C尿素呼气试验,观察组测定结果含量为95.15 dpm/mmol±8.34 dpm/mmol,H.pylori根除率为87.9%,症状缓解率为97%,不良反应率为4.5%,临床治疗总有效率为97%,对照组14C尿素呼气含量测定结果为100.32 dpm/mmol±10.34 dpm/mmol,H.pylori根除率为63.6%,症状缓解率为93.9%,不良反应率为18.2,总有效率为83.3%,观察组症状缓解率与对照组比较差异不明显,但是两组14C尿素呼气试验、H.pylori根除率、不良反应发生率、总有效率比较差异显著(P<0.05).结论:益生菌联合三联疗法对感染H.pylori的PU进行治疗,可以明显提高H.pylori根除率,不良反应少,患者治愈率明显增加,值得临床推广并应用.
AIM： To observe the effect of probiotics in the treatment of Helicobacter pylori（H. pylori） associated peptic ulcer（PU）. METHODS： One hundred and thirty-two patients with H. pylori associated PU treated at our hospital from May 2013 to May 2014 were randomly divided into two groups： an observation group（n = 66） and a control group（n = 66）.The observation group was given probiotics plus triple therapy, while the control group was given triple therapy alone. After one month of therapy,all patients underwent 14 C urea breath test to assess H. pylori infection status, and endoscopy was performed to assess ulcer healing. Clinical effects and improvement of gastrointestinal symptoms were compared between the two groups.RESULTS： After treatment, although the rate of symptom remission had no significant difference between the two groups（97% vs 93.9%, P 〉 0.05）, 14 C urea breath test value（95.15 dpm/mmol ± 8.34 dpm/mmol vs 100.32 dpm/mmol ± 10.34 dpm/mmol）, the rate of H. pylori eradication（87.9% vs 63.6%）, the rate of adverse reactions（4.5% vs 18.2%）, and the total effective rate（97% vs 83.3%） were significantly better in the observation group than in the control group（P 〈 0.05）.CONCLUSION： Probiotics combined with triple therapy can significantly improve the rate of H. pylori eradication, reduce adverse reactions, and increase the cure rate in patients with H. pylori associated PU.
