摘要
目的：研究血清β-HCG和孕酮对异位妊娠不同保守治疗方法中的预测价值。方法：选择2010年1月~2013年1月在临海市第二人民医院妇产科收治入院符合保守治疗条件的异位妊娠患者作为研究对象。根据治疗方案分为甲、乙两组,并回顾性选取异位妊娠行手术治疗的患者,分为腹腔镜手术和开腹术两组。结果：入组患者150例,甲组和乙组各75例。两组患者的年龄、BMI、孕次、产次、入组时β-h CG、孕酮水平、停经天数、包块大小和两组患者的保守治疗成功率等均无统计学差异（P〉0.05）;随着血清β-h CG（χ^2=63.59,P〈0.001）和孕酮（χ^2=20.41,P〈0.001）水平的升高,保守治疗的成功率有明显下降的趋势;手术组中,腹腔镜术后1天、7天患者β-h CG水平下降速度均显著快于开腹手术患者（P〈0.01）。结论：β-h CG和孕酮水平是异位妊娠不同保守治疗方案效果的重要预测因素,且β-h CG下降速度是患者术后恢复速度的重要指标。
Objective： To research the values of serum human β -chorionic gonadotropin （β -hCG） and progesteronei in predic- tion of different conservative therapies of ectopie pregnancy. Methods： A total of 150 patients with ectopic pregnancy and meeting the crite- ria of conservative therapy were selected from department of gynecology and obstetrics in this hospital from January 2010 to January 2013 as study objects, then the patients were divided into group A （75 patients） and group B （75 patients） according to therapeutic schedule; the patients with ectopic pregnancy and treated with surgery were selected retrospectively, then they were divided into laparoseopy group and lap- arotomy group. Results： There was no statistically significant difference in age, body mass index （BMI）, gravidity, parity, 13 -hCG and progesterone levels before treatment, the days of menopause, the size of mass and the successful rate of conservative therapy between the two groups （P 〉 0. 05 ） ; the successful rate of conservative therapy showed significant decreasing trend with the increases of serum 13 - hCG （X^2 = 63.59, P 〈 0. 001 ） and progesterone （X^2 = 20. 41, P 〈 0. 001 ） ; the decreasing speeds of serum β -hCG levels on the first day and the seventh day after laparoscopy were statistically significantly faster than those after laparotomy （ P 〈 0. 01 ） . Conclusion： β - hCG and progesterone levels are important predictive factors for effects of different conservative therapies of eetopie pregnancy, and the decreasing speed of serum β - hCG level is an important index of postoperative recovery.
