摘要
目的探讨结肠癌术后化疗患者发生感染的影响因素。方法选择2009年2月至2013年2月收治的286例结肠癌术后化疗患者为研究对象,采用自编问卷收集结肠癌术后化疗患者的临床资料,采用描述性统计和二分类Logistic回归分析结肠癌患者术后化疗感染的影响因素。结果结肠癌患者术后化疗感染发生率为14.7%(42/286),以上呼吸道感染最多见(33.3%),以革兰氏阴性菌感染为主(62.2%)。多因素Logistic回归分析显示,高龄(OR=3.022,P=0.000)、营养不良(OR=5.800,P=0.000)、侵入性操作>3种(OR=0.046,P=0.000)、血白细胞数低(OR=7.234,P=0.000)、糖化血红蛋白异常(OR=6.683,P=0.000)和陪护感染(OR=5.078,P=0.000)是结肠癌患者术后化疗感染的危险因素。结论结肠癌患者术后化疗感染发生率较高,以革兰氏阴性菌为主,其发生受多方面因素影响,在临床护理工作中,需规范医护操作行为,严格遵守无菌操作原则,加强陪护人员的管理,减少结肠癌术后化疗患者感染的发生。
Objective To investigate the influence factors of infection of patients with postoperative colorectal cancer chemotherapy. Methods 286 hospitalized patients with colorectal cancer from February 2009 to February 2013 were selected, and their clinical data were collected by the self-made questionnaire. The data were analysed by descriptive statistics and binary classification logistic regression. Results The infection rate was 14. 7% （42/286）,and the most was upper respiratory infection （33. 3% ）. Gram-negative bacte- ria （62. 2% ） was the main pathogenic bacteria. Multiple logistic regression analysis showed that venerable age （ OR = 3. 022,P = 0. 000）, malnutrition （ OR = 5. 800, P = 0. 000）, invasive operation kinds 〉 3 kinds （ OR = 0. 046, P = 0. 000） , low blood leukocyte count （ OR = 7. 234, P = 0. 000 ） , abnormal glycosylated hemoglobin （ OR = 6. 683, P = 0. 000） and the attendant infection （ OR = 5. 078, P = 5. 078 ） were independent risk factors. Conclusions The infection rate of patients with colon cancer admitted to postoperative chemotherapy remains high, and the pathogenic bacteria was mainly gram-negative bacteria. Various factors involved in infections. Preventive measures should focus on standardizing the medical operation, conforming the principle of aseptic operation, and strengthening the management of attendants to reduce infections.
