摘要
目的:构建以证候要素为内容的冠心病心绞痛中医疗效评价体系,并验证其可靠性。方法:利用100mm刻度法、综合集成研讨厅法,通过咨询全国23家三甲医院的130位专家,形成了冠心病心绞痛常见症状体征量化方案,并利用综合评价层次分析法及群组评判处理方法,结合专家咨询数据,以60个症状、体征为底层指标,以8个证候要素为第二层指标,以实证、虚证为第三层指标,以证候总积分为顶层指标,构建以证候要素为内容的冠心病心绞痛疗效评价体系。并收集了409例经冠脉造影证实的冠心病心绞痛患者来验证以证候要素为内容的冠心病心绞痛疗效评价体系。结果:构建的以证候要素为内容的冠心病心绞痛疗效评价体系与心绞痛周发作次数(Spearman系数为0.325,P=0.00)、生活质量(Spearman系数为0.285,P=0.00)有较强相关性。结论:构建的以证候要素为内容的冠心病心绞痛中医疗效评价体系,能体现中医治疗目标(证候)与结果(心绞痛周发作次数、生活质量)的相关性,可以用于冠心病心绞痛的中医疗效评价。
Objective： To construct a therapeutic effect evaluation system for patients with angina pectoris of coronary heart disease （CHD） based on syndrome factors in traditional Chinese medicine and verify its reliability. Methods： The quantization for common signs and symptoms of angina pectoris of CHD were established with 100mm calibration method, hal/ for workshop of metasynthetic engineering method and the opinions of 130 experts from national 23 of grade 3 and first-class hospitals. The therapeutic effect evaluation system was established by the 60 symptoms and signs as the underlying index, 8 syndrome factors as the second layer index and the excess syndrome and deficiency syndrome as the third level index, and the total score of symptoms and signs as the top level index. The evaluation system was constructed and improved by the comprehensive evaluation and level of analysis method, group evaluation method and data from expert consultation. And the therapeutic effect evaluation system was verified by coronary angiogram from 409 patients with angina pectoris of CHD. Results： There was a lose correlation between the therapeutic effect evaluation system and the frequency of angina attacks per week （Spearman coefficient=0.325, P=0.00） and quality of life （Spearman coefficient=0.285, P=0.00）. Conclusion： The therapeutic effect evaluation system for patients with angina pectoris of CHD based on syndrome factors in TCM could embody the correlation between the target （syndrome） and result （frequency of angina attacks per week and quality of life）, therefor which could be used for TCM therapeutic effect evaluation of angina pectoris of coronary heart disease.
